摘要 目的通过改变压力支持通气(pressure support ventilation,PSV)模式的压力上升速度(rate of pressure rise,Ramp),了解其对慢性阻塞性肺病(chronic obstructive pulmonary disease,COPD)患者呼吸力学以及吸呼切换的影响。方法 19例COPD患者采用PSV模式进行治疗,分析压力上升速度对流速-容量(Flow-Volume curve,F-V)曲线等呼吸力学参数的影响。结果压力上升速度越快,COPD患者在PSV模式时吸气肌用力就越小。压力上升速度与PSV模式的吸呼切换无关。结论对于老年COPD患者,使用0.2的Ramp较好。 Objective To evaluate the effects of pressure ramp slop values on respiratory mechanics and inspiration termination criteria during pressure support ventilation (PSV). Methods A total of 19 COPD patients were treated by PSV. Effect of ramp slop values on respiratory parameters including Flow- Volume (F-V) curve was analysed. Results The faster the pressure was increased,the less strength of the inspiratory muscle of COPD patients used during PSV. There was no relation between the increasing speed of pressure and shift of PSV modes. Conclusion For elderly COPE) patients, using Ramp at 0. 2 is the best.
