双钢板治疗肱骨中下段骨折术后骨不连

Two-plate fixation for nonunion of mid-distal humeral fracture

目的 探讨采用双钢板内固定加自体植骨治疗肱骨中下段骨折术后骨不连的疗效。方法 自2006年7月至2011年10月共收治11例肱骨中下段骨折术后骨不连患者,全部采用LCP＋重建钢板＋自体植骨治疗,术后不用外固定,肘关节早期功能锻炼。结果 手术切口均一期愈合,随访时间6~18个月,平均9.6个月。骨折均愈合,愈合时间4~8个月,平均6.4个月,无内固定失败病例。依据ConstantMurley评分,11例患者术前肩关节评分 22.6±4.7,术后6个月评分72.5±6.8。结论 采用LCP＋重建钢板＋自体植骨技术是治疗肱骨中下段骨折内固定失败后骨不连的一种有效方法。

Objective To evaluate the treatment for nonunion of mid-distal humeral fracture with two-plate fixation and autogenous bone graft. Methods Eleven patients （average age,forty-two years） who had an unstable,ununited fracture of the mid-distal humeral were managed with two-plate fixation and application of an autogenous bone graft from the iliac crest. Results The average follow-up was 9.6 months （range 6-18 months）.The fracture united in all eleven of the patients.There was no loosening or breakage of a fixation device.According to ConstantMurley scoring system,the mean score was 22.6＆plusmn;4.7 before the surgery,and improved to 72.5＆plusmn;6.8 6month after the surgery. Conclusions Two-plate fixation and autogenous bone graft for the treatment of mid-distal humeral nonunion presents satisfactory results.
