乌拉地尔与硝酸甘油联用治疗急性左心衰竭伴高血压的临床研究

Clinical Study of Urapidil Combined with Nitroglycerin on Patients with Acute Heart Failure and Hypertension

目的探讨乌拉地尔联合硝酸甘油治疗急性左心衰竭伴高血压的临床效果。方法选取我院2010年2月至2012年10月收治急性左心衰竭伴高血压患者90例,采用随机数字表法分为对照组和联合治疗组,每组各45例;其中对照组患者采用硝酸甘油静脉滴注治疗;联合治疗组患者在对照组治疗基础上,加用乌拉地尔静脉泵入治疗;比较两组患者临床改善总有效率,治疗前后血压、心率及血氧饱和度等心功能指标。结果对照组和联合治疗组患者临床改善总有效率分别为68.9%,86.7%;联合治疗组患者临床改善总有效率明显高于对照组,组间比较差异显著(P<0.05);对照组与联合治疗组患者治疗前血压、心率及血氧饱和度等心功能指标组间比较无显著差异(P>0.05);两组患者治疗后血压、心率及血氧饱和度等心功能指标较治疗前明显改善,且联合治疗组患者改善程度明显优于对照组,组间比较差异显著(P<0.05)。结论乌拉地尔联合硝酸甘油治疗急性左心衰竭伴高血压能够显著缓解临床症状,提高心脏功能,改善生活质量,具有临床使用价值。

Objeetive To investigate the therapeutic effect ofurapidil combined with nilroglycerin on patients with acute heart failure and hypertension. Methods 90 patients with acute.heart failure and hypertension were chose in the period from February 2010 to October 2012 in our hospital and randomly divided into two groups including control group （45 patients） with nitroglycerin inWavenous treatment and observation group （45 patients） with umpidi inlravenous pump treatment on the basis of conlml group; and the clinical treaWaent efficiency, the heart function index including blood pressure, heart rate and oxygen saturation before and after Ireatment of both groups were compm~ Results The clinical treatment efficiency of control group and observation group were separately 68.9%, 86.7%; The clinical treatment efficiency of observation group were significantly higher than congol group （P〈0.05）. There was no significant difference in the heart function index including blood pressure, heart rate and oxygen saturation before Ireatment between control group and observation group （P〉0.05）; the heart function index including blood pressure, heart rate and oxygen saturation after treatment of observation group were significantly better than control group （P〈0.05）. The improving degree of heart function index including blood pressure, heart rate and oxygen saturation after treatment of observation group were significantly better than control group （P〈0.05）. Conclusion Urapidil combined with nitroglycerin on patients with acute heart failure and hypertension can efficiently improve clinical symptoms and heart functlorg higher life quality, and is worthy of clinical appficatlon value.
