中枢神经细胞瘤的CT和MRI表现

CT and MRI findings of central neurocytoma

目的探讨中枢神经细胞瘤（CNC）的CT和MRI表现，提高对本病的认识。方法分析2008年2月～2013年2月期间在我院神经外科收治的7例CNC患者的CT和MRI检查资料和病历资料。结果全部7例肿瘤均位于侧脑室内，与透明隔分界不清，病变呈不规则形肿块影，周边可有分叶，肿块前后径较长，左右径较短。CT平扫显示7例肿块实质与脑组织呈等或稍高密度，6例肿块内部见囊变区和钙化影，病灶周边清楚，增强扫描肿块实质部分呈中度或强化，囊变区无强化；MRI平扫显示6例肿瘤由囊性部分和实性部分组成，1例由实性部分组成，实性部分与周围脑组织呈等或稍高信号，囊性部分与脑脊液呈等信号，肿瘤内部囊变区之间见丝网状分隔．注射造影剂后，肿瘤实性部分明显强化，囊性部分无强化。结论中枢神经细胞瘤CT和MRI表现有一定特点，综合分析患者临床和影像学表现，能够提高诊断准确率，确诊仍需病理学检查。

Objective To investigate the CT and MRI characteristics of central neurocytoma （CNC） in order to improve our knowledge of this disease. Methods Retrospectively analyzed CT and MRI data and medical records of 7 patients with CNC hospitalized in the Department of neurosurgery in our hospital from February 2008 to February 2013. Re- suits All the tumors of 7 patients were located in the lateral ventricles and it had no clear demarcation with septum pellucidum, the lesion showed irregular mass, surrounding a lobulated mass, long anteroposterior diameter, short trans- verse diameter, tumor essence and brain tissue of 7 patients showed iso-or hyper-density on CT examination, cystic areas and calcification were seen in the internal masses of 6 patients, the lesions showed a sharp borders, substantial part of the masses showed moderate or strengthening enhancement after intensified scanning, and no enhancement in cystic areas; Cystic and solid parts were in tumors of 6 cases after MRI scanning, only solid part was in 1 case, equal or slightly higher signal were on the solid part and the surrounding brain tissue, equal signal on cystic part and CSF, reticular septation were seen between intratumoral cystic areas, after injection of contrast agent, the solid part of tumor had enhancement, while cystic part had no enhancement. Conclusion CT and MRI findings of central neurocytoma have certain characteristics, it can improve the accuracy rate of diagnosis to analyze the clinical and imaging findings comprehensively, but it needs pathological examination to diagnose.
