粪石性肠梗阻1例报告

Stercoral ileus: A case report

目的探讨海南地区粪石性肠梗阻成因、临床表现及治疗方法。方法对2012年5月我院海南分院收治的1例粪石性肠梗阻的临床资料、手术方法及治疗经验进行分析,并进行相关文献复习。结果患者入院后经保守治疗无效,在全麻下接受腹腔镜探查术、小肠切开粪石取出术,术后16 d再次出现梗阻,肠镜下明确为粪石梗阻,术后23 d二次行剖腹探查、小肠切开粪石取出术。患者二次术后第3天恢复胃肠道功能,第9天进固体食物,第13天痊愈出院,未出现手术并发症。结论进食番石榴可能为海南地区粪石性肠梗阻的主要原因,诊治应注意选择合理的手术方式,防止并发症发生。

Objective To study the causes, clinical features and treatment of stercoral ileus in Hainan Province. Methods Clinical data, surgical procedures and treatment of 1 stercoral ileus patient admitted to our hospital in May 2012 were retrospectively analyzed with the related literature reviewed. Results The patient had no response to conventional treatment after admission and thus underwent laparoscopy-assisted exploration with the bezoar removed under general anesthesia. However, the patient received abdominal laparotomy on day 23 after admission due to the recurrence of ileus 16 days after operation. The gastrointestinal function of the patient was recovered 3 days after operation. The patient had solid food 9 days after operation and was discharged 13 days after opera tion without complications. Conclusion Eating guava may be the major cause of stercoral ileus in Hainan Province, attention should thus be paid to its surgical procedure and complications.
