摘要
目的观察当归须散治疗急性肺挫伤的疗效及其对患者血清炎性因子水平的影响。方法肺挫伤患者80例,随机分为对照组和治疗组各40例。两组均给予常规治疗,治疗组在此基础上加用当归须散治疗,疗程均为2周。观察两组肺挫伤后继发性肺损伤情况和康复时间,血清肿瘤坏死因子(TNF-α)、IL-6,C反应蛋白(CRP)水平变化。结果与对照组比较,治疗组肺部感染率、全身炎症反应综合征(SIRS)发生率、急性呼吸窘迫综合征(ARDS)发生率、病死率低,临床疗效总有效率高,康复时间短(P<0.05或<0.01)。与对照组治疗后比较,治疗组TNF-α、IL-6、CRP水平低(P均<0.01)。结论当归须散能显著降低急性肺挫伤患者炎性细胞因子TNF-α、IL-6、CRP水平,抑制炎症反应,阻断肺损伤的演变进程,缩短康复时间,对肺挫伤有明显的治疗作用。
Objective To observe the efficacy of Dangguixusan decoction in treatment of acute pulmonary contusion and its effect on serum levels of inflammatory cytokines. Methods Eighty patients with acute pulmonary contusion were randomized into 2 groups, 40 in each: the control group and the treatment group. Patients in the control group were given routine treatment, while patients in the treatment group were given Dangguixusan decoction besides routine treatment for 2 weeks. Secondary pulmonary injury, recovery time and the serum levels of tumor necrosis factor (TNF)-α and interleukin- 6(IL-6), C-reactive protein (CRP) were observed in two groups. Results Compared with the control group, the rate of pulmonary infection, the incidences of systemic inflammatory response syndrome ( SIRS), acute respiratory distress syn- drome ( ARDS), and fatality rate were decreased, meanwhile, the total effective rate of clinical efficacy was increased, and the recovery time was shortened in the treatment group (P 〈0. 05 or 〈 0.01 ). In addition, the serum levels of TNF-α, IL- 6 and CRP in the treatment group were decreased as compared with the control group after treatment ( all P 〈0.01 ). Con- clusion Dangguixusan decoction can significantly decrease the serum levels of TNF-α, IL-6 and CRP in patients with a- cute pulmonary contusion, inhibit the inflammatory reaction, block the progress of contusion, shorten the recovery time, and has the obvious therapeutic effect on pulmonary contusion.
