负压封闭引流术联合关节腔灌洗治疗骨折术后膝关节感染的疗效

Efficacy of vacuum sealing drainage combined with joint cavity lavage in treatment of knee infections after fracture surgery

目的通过分析骨折内固定手术后膝关节内感染患者临床治疗资料,探讨改良负压封闭引流手术联合关节腔灌洗方法治疗术后膝关节感染的临床效果。方法选取2011年6月-2012年6月收治的椎管髓外膜内肿瘤患者40例,按照治疗方法分为试验组与对照组,分别进行改良负压封闭引流手术联合关节腔灌洗方法治疗与常规静脉给药治疗,探讨试验组治疗方法的临床效果,并对比分析使用两种治疗方法抗感染的效果。结果试验组患者经改良负压封闭引流手术联合关节腔灌洗治疗后,各类骨折与切口均成功愈合,无复发性感染症状,与治疗前相比切口感染与关节内感染以及关节囊分裂等均有显著改善(P<0.05);试验组患者的膝关节关节内感染治疗成功率达95.0%,对照组为70.0%,两组对比差异有统计学意义(P<0.05)。结论采用改良负压封闭引流手术联合关节腔灌洗方法治疗骨折内固定手术后膝关节关节内感染患者,具有方法简单、创伤小、安全可靠性高、患者痛苦少、负担轻的优势,临床效果显著。

OBJECTIVE To obserue the clinical data of knee joint infections after line fracture fixed surgery and study the clinical effect of improved negative pressure closed drainage joint articular cavity lavage in the treatment of the infections after knee joint fractures. METHODS The spinal cord adventition tumor patients who were treated in the hospital from Jul 2011 to Jul 2012 were enroued in the study, according to the treatments,40 patients were averagely divided into the experimental group and control group and were given improved negative pressure cIosed drainage operation joint articular cavity lavage treatment and conventional intravenous drug treatment respectively. Then, we analyzed the clinical and anti-infective effect of two kinds treatments. RESULTS Patients in the experiment group got much improvement after treatment. All kinds of fracture and incision healing were successful, no recurrent infection symptoms. And compared with before treatment, incision infections, intra- articular infections and articular capsule division were significantly decreased （P^0. 05）. The success rate of curing infection in experiment group was 95.0%, significantly higher than 70. 0% of the control group, the difference between the two groups was statisticully significant （P % 0. 05）. CONCLUSION Modified vacuum sealing drainage combined with joint articular cavity lavage treatment on the fracture fixed surgery in patients with knee joint infections, possesses several advantages, such as simplicity, small trauma, safety, high reliability and the patient＇s pain burden light, moreover the clinical effect is remarkable.
