摘要
目的探讨高危型人乳头状瘤病毒(HPV)感染检测宫颈癌在临床检测中的作用,以提高宫颈癌的检出率。方法 2012年6-12月采用第二代杂交捕获试验(HCⅡ)对69例宫颈癌、54例宫颈上皮内瘤变及41例宫颈炎高危型HPV DNA进行检测。结果高危型HPV-DNA在宫颈癌中的表达阳性率为59.4%,明显高于正常宫颈上皮的7.3%和宫颈上皮内瘤变的表达率13.0%,差异均有统计学意义(P<0.05);宫颈癌中肿瘤≥4cm者高危型HPV-DNA阳性率为90.3%,明显高于<4cm者的25.5%(P<0.05),FIGO分期Ⅲ~Ⅳ期者HPV16/18阳性率明显高于<4cmⅠ~Ⅱ期者(P<0.05);高危型HPV-DNA的表达与患者年龄、细胞分化、淋巴结转移无关。结论高危型HPV与宫颈癌的发生、发展密切相关,通过高危型HPV-DNA的检测,可明显提高宫颈癌的检出率,对降低宫颈癌的发生率和病死率具有重要意义。
OBJECTIVE To discuss the role of high risk human papillomavirus (HPV) infections in clinical detection of cervical cancer so as to improve the detection rate of cervical cancer. METHODS From Jun 2012 to Dec 2012, the high risk HPV DNA were detected in 69 cases with cervical cancer, 54 cases with cervical intraepithelial neoplasia, and 41 cases with cervicitis by using the second generation hybrid capture test(HC). RESULTS The positive rate of high risk HPV DNA in the patients with cervical cancer was 59.4~, significantly higher than 7.3o//oo of the patients with normal cervical epithelium and 13. 0% of the patients with cervical intraepithelial neoplasia, the difference was significant(P^0.05). The positive rate of high risk HPV DNA in the cervical cancer patients with the tumor size more than 4cm was 90.3 ~, significantly higher than 25.5 ~ of the patients with the tumor size less than 4cm(P^0.05) ; the positive rate of HPV16/18 in the patients with the FIGO stage varying between ~ and IV was significantly higher than that of the patients with the FIGO stage varying between I and ]1 (P~ 0. 05). The expression of the high risk HPV DNA was negatively correlated with the age, cell differentiation, and lymph node metastasis. CONCLUSION The high risk HPV DNA has a close relationship with the occurrence and development of cervical cancer; the detection of high risk HPV DNA can obviously improve the detection rate of cervical cancer, which has great significance in reducing the incidence of cervical cancer and the mortality of the patients.
