重症监护室多重耐药铜绿假单胞菌产OXA酶基因的研究 被引量:8

摘要 目的探讨本地区重症监护室多重耐药铜绿假单胞菌对β-内酰胺类抗生素的耐药特点及其OXA型耐药基因的分析。方法采用K-B法检测32株多重耐药铜绿假单胞菌对4种β-内酰胺类抗生素(氨曲南、头孢吡肟、亚胺培南和哌拉西林/三唑巴坦)的耐药情况;采用多重PCR法检测32株多重耐药铜绿假单胞菌的4种OXA型基因(OXA23、OXA24、OXA51和OXA58)。结果 32株多重耐药铜绿假单胞菌对β-内酰胺类抗生素耐药率分别为:氨曲南87.5%(28/32)、头孢吡肟62.5%(20/32)、亚胺培南59.38%(19/32)、哌拉西林/三唑巴坦56.25%(18/32)。32株多重耐药铜绿假单胞菌中有2株分别含bla OXA23基因和bla OXA51基因。结论重症监护室多重耐药铜绿假单胞菌对β-内酰胺类抗生素的耐药率较高,均在50%以上。bla OXA51基因在本地区首次检出,但其播散流行趋势可能不如bla OXA23基因。 Explore the features of 13-1actam antibiotic resistance and analyse OXA-type gene in multidrug-resistant Pseudomonas aeruginosa isolated from intensive care unit in this region. Methods K-B test was used to detect the rate of drug resistance in 32 multidrug-resistant Pseudomonas aeruginosa resisted to 4 13-1actam antibiotics(Aztreonam,Cefepime, Imipenem and Piperacillin/Tazobactam). 4 kinds of OXA-type genes(blaoxA23, blaoxa24, blaoxA51 and blaoxn58)were detected by multiplex PCR in 32 multi-drug resistant Pseudomonas aeruginosa. Results The rates of drug resistance in 32 multidrug-resistant Pseudomonas aeruginosa to 4 kinds of 13-1actam antibiotics were respectively aztreonam 87.5%(28/32),cefepime 62.5%(20/32), imipenem 59.38%(19/32), piperacillin/ tazobactam 56.25%(18/32); In 32 isolates of multidrug-resistant Pseudomonas aeruginosa, a strain contained blaoxA23 genes and another contained blaoxn51 gene. Conclusion β-1actam antibiotic resistance rate was higher than 50% in multi-drug resistant Pseudomonas aeruginosa isolated from intensive care unit. blaonoxa51 gene was detected for the first time, but its spreading tendency may not be so widely compared with blaoxa23 gene.
