糖皮质激素高剂量雾化吸入在重度哮喘急性发作患儿中的应用价值 被引量:13

摘要 目的:探讨重度哮喘急性发作(简称哮喘)患儿雾化吸入高剂量糖皮质激素(ICS)的临床疗效及安全性。方法:选取42例哮喘患儿为研究对象,根据随机数字表将其分为高剂量ICS组(观察组)22例以及常规剂量组20例(对照组),对比分析两组患儿治疗前后临床症状评分、肺功能指标、炎症介质(IL-8、IL-17)水平以及嗜酸粒细胞离子蛋白(ECP)、中性粒细胞过氧化物酶(MPO)。结果:观察组后哮喘日间症状评分、夜间症状评分显著低于对照组日间症状评分及夜间症状评分,差异有统计学意义(P<0.05)。与治疗前相比,两组患者肺功能指标均得到相应改善,而治疗后观察组FEV1.0[(92.3±16.2)%]、PEF[(88.5±23.5)%]以及FEV1.0/FVC[(83.6±14.8)%]显著高于对照组[(58.3±11.2)%、(54.8±11.3)%、(65.3±13.8)%],差异有统计学意义(P<0.05)。与治疗前相比,两组患儿炎症因素水平得到改善,治疗后观察组IL-8[(107.8±22.9)ng/L]、IL-17[(147.4±38.5)ng/L]、嗜酸粒细胞[(0.11±0.08)%]、中性粒细胞[(0.26±0.10)%]显著低于对照组,差异有统计学意义(P<0.05)。结论:对于重度急性哮喘发作患儿应用高剂量ICS雾化吸入可有效改善患儿肺功能指标及炎症因子水平,可快速缓解急性哮喘发作期患儿的临床症状,有利于患儿预后。
