重症医学科护士血压及隐性高血压患病率调查

Investigation of blood pressure and masked hypertension in ICU nurses

目的:调查重症监护(ICU)科室护士诊室血压(OBP)、动态血压监测(ABPM)数据及隐性高血压(MH)患病状况。方法:选择参加2011年解放军第463医院和沈阳军区总医院春季体检中ICU护士,63例及门诊科室护士63例,分别为ICU护士组和门诊对照组,两组进行1∶1配对,配对条件为:相同性别、年龄及护龄、目前岗位从业时间、职称、文化程度和人体质量指数。两组均接受了OBP和24h-ABPM检测。结果:OBP测量值:与门诊对照组比较,ICU护士组的OSBP[(131.56±20.38)mmHg比(134.73±21.35)mmHg]和ODBP[(80.73±15.29)mmHg比(84.29±16.40)mmHg]显著升高(P<0.05);24h-ABPM各类监测指标:与门诊对照组比较,ICU护士组的24h平均收缩压、舒张压,白天收缩压、舒张压,夜间收缩压、舒张压测量值均显著升高(P<0.05或<0.01);同时,ICU护士组的MH患病率显著高于门诊对照组(28.57%比11.11%,χ2=6.047,P<0.05)。结论:大医院ICU科室护士诊室血压及24h动态血压监测各类指标显著升高,且隐性高血压患病率显著增加。

Objective： To investigate office blood pressure （OBP）, ambulatory blood pressure monitoring （ABPM） da-ta and morbidity of masked hypertension （MH） in nurses working in intensive care unit （ICU）. Methods： The 63 ICU nurses and 63 out-patient nurses who received physical check-up in 463^rd Hospital and general hospital of Sheny-ang military command in Chinese PLA in spring 2011were respectively regarded as ICU group and control group. The 1 ： 1 pairing method was used for two groups, and the pairing conditions were： same gender, age, nursing age, occupation time on the present job, professional title, education and body mass index （BMI）. Both groups received measurements of OBP and 24h-ABPM. Results： Compared with outpatient control group, there were significant in-crease in office systolic blood pressure [OSBP, （131.56 ± 20.38） mmHg vs. （134.73 ± 21.35） mmHg] and office di-astolic blood pressure EODBP, （80.73 ± 15.29） mmHg vs. （84.29 -± 16. 40） mmHg] in ICU nurse group, P〈0.05; there were significant increase in 24h mean SBP and DBP, daytime SBP and DBP, nighttime SBP and DBP in ICU nurse group, P〈0. 05 or 〈0. 01; meanwhile, incidence rate of MH in ICU nurse group （28.57%） was significantly higher than that of outpatient control group （11.11%）, X^2 = 6. 047, P〈0.05. Conclusion： Office blood pressure and indexes of 24h ambulatory blood pressure monitoring are higher and incidence rate of masked hypertension signifi- cantly increases in ICU nurses of large hospitals,
