摘要
目的在上海市闵行区儿保门诊开展婴幼儿先天性心脏病(CHD)筛查,了解CHD患病情况,探索在儿童保健门诊开展CHD筛查的有效方法。方法在2011年8月1日—2012年12月31日期间对2009年1月1日—2011年12月31日出生的闵行区户籍儿童,在其首次到儿保门诊体检时,由社区儿保医生完成一次CHD七项指征筛查,筛查医生将结果录入闵行区儿童电子健康档案,并将筛查结果为阳性的儿童转诊至三级专科医院,定期随访,了解其CHD的确诊情况。结果上海市闵行区户籍儿童CHD初筛阳性率为4.34‰(103/23 717),筛查阳性的儿童在三级专科医院进行彩色多普勒超声心动图检查后,CHD确诊率为68.32%(69/101),其中以室间隔缺损居多,占37.68%(26/69),儿保门诊CHD患病率为2.91‰(69/23 717)。结论上海市闵行区户籍婴幼儿CHD患病率与全国其他地区相似。开展此项筛查,首先应确保社区儿童保健医生充分掌握CHD七项指征的筛查技术,同时需创造良好的心音听诊环境;此外,有必要对儿童家长开展CHD的健康宣教工作。
OBJECTIVE To figure out the general situation of Congenital Heart Disease（CHD） among the children in children insurance clinics of Minhang District, so as to explore the method and procedure related to CHD screening. METHOD Children born between Jan 1st, 2009 to Dec 31th, 2011 were checked during Aug 1st, 2011 to Dec 31th, 2012 applying for 7 CHD indicators. All the participants were applied to CHD screening when first visited child health department, and then the results were put into the eHR personal health file. RESULTS The positive screen rate of CHD among the residents children of Minhang District is 4.34%o（103/23 717）, the CHD positive rate for children after color Doppler ultrasound cardiogram is 68.32%（69/101）, the rate for ventricular septal defect is 37.68%（26/69）, the CHD prevalence rate in children health care clinic is 2.91%o（69/23 717）. CONCLUSION The prevalence rate of CHD among the resident children of Minhang District is similar to the other districts. Considering the cost and effect of 7 indicators related to diagnosis of CHD, a routine medical checkup of CHD with 7 indicators method should be recommended, and a systematic CHD diagnostic procedure and health education should be built up and performed.
