正压通气治疗高血压伴OS-AHS 24例临床疗效观察

摘要 目的:探讨正压通气对高血压伴阻塞性睡眠呼吸暂停低通气综合征(OS-AHS)患者的临床疗效。方法:选取本院收治的48例高血压伴OS-AHS患者,随机均分为对照组(常规治疗)和观察组(在常规治疗的基础上进行正压通气治疗),比较两组患者治疗前后的收缩压(SBP)、舒张压(DBP)、脉压(PP)、心率(HR)的变化及睡眠呼吸暂停低通气指数(AHI)、最长呼吸暂停时间和最低脉搏容积血氧饱和度(SpO2min)的差异。结果:两组患者治疗后病情均得到不同程度的改善,同治疗前比较差异有统计学意义(P<0.05);观察组治疗后SBP、DBP、PP、HR、AHI、最长呼吸暂停时间和SpO2min较对照组改善更显著(P<0.05)。结论:正压通气治疗高血压伴OS-AHS患者安全有效,可有效降低血压,改善患者睡眠缺氧状态。 Objective:To investigate efficacy of positive pressure ventilation for the treatment of hypertension with obstructive sleep apnea-hypopnea syndrome(OS-AHS).Method:48 cases of hypertension with OS-AHS in our hospital were randomly divided into control group(received conventional treatment) and observation group (received positive pressure ventilation based on conventional treatment).The systolic blood pressure(SBP),diastolic blood pressure(DBP),pulse pressure(PP),heart rate(HR),sleep apnea-hypopnea index(AHI),the longest apnea time and minimum pulse volume of oxygen saturation(SpO2min) of two groups before and after treatment were compared.Result:All patients were improved to varying degrees after treatment than before treatment(P〈0.05);The SBP,DBP,PP,HR,AHI,the longest apnea time and SpO2 min of observation group were improved more significantly than control group(P〈0.05).Conclusion:Positive pressure ventilation for the treatment of hypertension with OS-AHS is safe and effective.It can effectively lower blood pressure and improve sleep hypoxic state.
