静脉吸毒HIV-1感染者Th17/Treg失衡与疾病进展及单核细胞活化水平的相关性研究 被引量:5

摘要 目的了解静脉吸毒HIV-1感染者Th17和Treg细胞百分比及Th17/Treg比例平衡状态,分析其与疾病进展和单核细胞免疫活化的相关性,探讨Th17/Treg比例失衡在HIV致病机制中的作用。方法选取经静脉吸毒感染HIV-1的患者58例,采集静脉血,分离外周血单个核细胞(PBMC),流式细胞术检测Th17和Treg百分比及Th17/Treg比例。ELISA方法检测血浆中sCD14水平。结果 HIV-1感染者Th17细胞百分比和Th17/Treg比值明显低于健康对照组,差异均有统计学意义(P均<0.01),Th17百分比和Th17/Treg比值与CD4数量呈正相关(P<0.01,r=0.46;P<0.01,r=0.47),与病毒载量呈负相关(P<0.01,r=-0.65;P<0.01,r=-0.69)。HIV-1感染者血浆sCD14水平显著高于健康对照组组(P<0.01),且Th17百分比和Th17/Treg比值与血浆sCD14水平呈负相关(P<0.01,r=-0.54;P<0.01,r=-0.49)。结论 HIV-1感染吸毒人群中Th17和Treg百分比和Th17/Treg比值均明显失衡,且与病毒载量和单核细胞活化标志sCD14均呈负相关,提示Th17/Treg失衡与HIV感染所致病毒复制和免疫活化密切相关。 Objective To study Th17, Treg, Th17/Treg ratio and the association with disease progression and monocyte activation in HIV-1 infected intravenous (iv) drug users, and to explore the mechanism of Th17/Treg imbalance. Methods A total of 58 HIV-1 infected iv drug users (IDUs) were recruited and their peripheral blood mononuclear cells (PBMCs) were isolated and analyzed for the frequency of Thl7, Treg and the ratio of Th17/Treg by flow cytometry. Plasma level of sCD14 was detected by ELISA. Results Comparing to healthy controls, Th17 cells and Th17/Treg ratio were decreased in HIV 1 infected IDUs (P〈0. 001); and both are positively correlated with CD4 counts (P〈0.001, r=0.46 and P〈0.001, r=0.47, respec- tively) and negatively correlated with plasma viral load (P〈0. 001, r=-0.65 and P〈0. 001, r=-0.69, re spectively) . Plasma level of sCD14 was increased in HIV-1 infected IDUs, and negatively correlated with ei ther Thl7 frequency or Th17/Treg ratio (P〈0. 001, r =-0.54 和 P〈0. 001, r=-0.49, respectively). Con- clusions Imbalance of Th17/Treg ratio was observed in HIV-1 infected iv drug users, and this ratio is associated with disease progression and monocyte activation, suggesting a role of Th17/Treg in HIV pathogenesis.
