背景:一些研究表明脂代谢和血钙异常可能与结直肠腺瘤(CRA)和结直肠癌(CRC)有一定关联。目的:明确血脂、血钙水平与结直肠肿瘤的相关性。方法:回顾性收集2011年1月~2012年10月于新疆医科大学第五附属医院行结肠镜检查并符合入选条件者的病史资料,根据内镜和活检病理检查结果将入选者分别归入CRA组(n=84)、CRC组(n=68)和对照组(n=88)。分析三组间血脂(TC、TG、LDL-C、HDL-C、apoAⅠ、apoB)、血钙水平的差异,血脂、血钙水平与CRA、CRC的相关性以及血钙水平与各血脂指标的相关性。结果:CRA、CRC患者存在明显血脂、血钙异常。高TC、高TG、高LDL-C、低HDL-C、低apoAⅠ、低血钙与CRA相关(P<0.05),低TC、高TG、低HDLC、低血钙与CRC相关(P<0.05)。CRA、CRC患者的血钙水平均与HDL-C呈正相关(P<0.05)。结论:高TG、低HDL-C、低血钙与结直肠肿瘤的发生有一定相关性,TG可能为促进因素,HDL-C和血钙可能为保护因素。
Background: It has been revealed that abnormalities in lipid metabolism and blood calcium were associated with colorectal adenoma (CRA) and colorectal carcinoma (CRC). Aims: To determine the association between serum lipid, blood calcium and colorectal neoplasms. Methods: A retrospective analysis was carried out in patients undergoing colonoscopy and fulfilling the inclusion criteria in the Fifth Affiliated Hospital of Xinjiang Medical University from Jan. 2011 to Oct. 2012. Patients enrolled in the study were assigned into CRA group (n=84), CRC group (n=68) and control group (n=88) referring to the endoscopic findings and results of biopsy pathology. Differences in serum lipid (TC, TG, LDL-C, HDL-C, apoAⅠ and apoB) and blood calcium in these three groups, relations of serum lipid and blood calcium with CRA and CRC, as well as relations of blood calcium with various serum lipid indices were analyzed. Results: Significant abnormalities in serum lipid and blood calcium were seen in patients with CRA and CRC. Higher TC, TG, LDL-C, lower HDL-C, apoAⅠ and lower blood calcium were positively correlated with CRA (P〈0.05), while higher TG, lower TC, HDL-C and lower blood calcium were positively correlated with CRC (P〈0.05). Blood calcium were positively correlated with HDL-C in patients with CRA and CRC (P〈0.05). Conclusions: Higher TG, lower HDL-C and lower blood calcium might be involved in the pathogenesis of colorectal neoplasms. Among them, TG may act as a promoting factor and HDL-C and blood calcium as protective factors.
