子宫脱垂伴阴道前后壁膨出阴式手术效果临床观察

Clinical effect observation of vaginal hysterectomy for uterine prolapse with anteroposterior prolapse

目的观察分析对子宫脱垂伴阴道前后壁膨出患者进行阴式子宫切除术的结果，探讨其临床效果。方法将本院于2008年1月～2013年3月接收的41例子宫脱垂伴阴道前后壁膨出患者临床资料作为其研究对象。根据患者意愿及按照手术方法不同分为使用单纯阴式子宫切除术组（对照组）与腹腔镜辅助阴式子宫切除术组（试验组）。针对两组治疗后临床疗效、并发症发生等综合情况进行对比分析研究。结果试验组（腹腔镜辅助）患者手术出血量、手术时间以及住院时间、并发症发生几率和排气时间等临床综合指标均低于对照组（单纯阴式子宫切除），差异具有统计学意义（P〈0．05）。结论腹腔镜辅助阴式子宫切除术与单纯阴式子宫切除术对比，具备更好的手术视野，效果较好，优点较多，出血量较少，患者住院时间短，同时并发症发生几率较低，相对而言是一种优异的子宫切除方式，安全有效，在临床上值得进一步推广。

Objective To observe and explore the clinical effect of vaginal hysterectomy for the patients of uterine prolapse with anteroposterior prolapse. Methods The clinical data of 41 cases of uterine prolapse with anteroposterior prolapse in our hospital from January 2008 to March 2013 were received and selected as the object of study.According to the wishes of patients and different surgical methods,the patients were divided into simple vaginal hysterectomy group（control group） and laparoscopically assisted vaginal hysterectomy group（experimental group）.The clinical efficacy and incidence of complications of two groups after the treatment were compared and analyzed. Results The clinical integrated indicators such as surgical blood loss,operative time,hospital stay, incidence of complications and exhaust time of the experimental group（laparoscopic assisted vaginal hysterectomy） were lower than those of the control group （simple vaginal hysterectomy）,and the differences were statistically significant（P 〈 0.05）. Conclusion Compared with simple vaginal hysterectomy,laparoscopic assisted vaginal hysterectomy has better surgical field,better effects, more advantages,less bleeding, shorter hospital stay,and lower incidence of complications.Comparatively speaking,it is an excellent way to hysterectomy,safe and effective,and worthy of further clinical promotion.
