摘要
目的初步探讨肝癌和门脉癌栓DwI和ADC表现。材料和方法报道36例肝癌(45个癌灶)伴门脉癌栓(39支),所有患者均作肝脏MRI平扫、DWI和ADc成像、多期增强或动态扫描。结果在增强动脉期,有41个癌灶和32支门脉癌栓强化。在DwI和ADC图上,癌灶表现高、低信号的分别有43和35个,PVTT表现为高、低信号的分别有37个和30支。癌灶和PVTT的DWI、ADC表现与癌灶和PVTT的动脉期强化表现比较,无显著差异(P〉0.05)。癌灶和PVTTADC值分别为(1.134±0.205)×10-3mm2/s和(1.016±0.473)×10-3mm2/S,无显著差异P〉0.05:癌灶和PVTT的ADC值双侧95%置信限分别为1.062~1.206×10-3mm2/s和0.909~1.123×10-3mm0/S,无显著差异P〉0.05。结论PVTT与肝癌的DWI和ADC表现相似,这些表现可以为肝癌和PVTT的MRI诊断提供参考。
Objective To explore the findings of diffusion-weighted imaging(DWl) & appar- ent diffusion coefficient(ADC) in the hepatocellular carcinoma with the portal vein tumor thrombus (PVTT). Materials & Methods 36 cases of HCC (45 lesions) with PVTT (39 branches) were reported, 36 cases were made by non-enhancement & DWI & ADC & MRI multiphase enhancement or dynamic scan. Results 41 of 45 HCCfocuses & 32 of 39 branches of PVTT were enhanced dnring the arterial phase of enhancement. 43 & 35 of HCC focuses & 37 & 30 branches of PVTT were respectively showed a high & low signal on DWI and ADC images in the group. The incidence of MRl findings of low signal on ADC & high signal on DWI & enhanced on the arterial phase were showed no significant differ- ence (p 〉 0.05) between the HCC lesions & PVTT. The values of ADC of HCC focuses & PVTT were respectively as (1.134 ± 0.205) × 10-3mm-2/s & (1.016 ±0.473) × 10-3mm-2/s & between the values of ADC of the HCC & PVTT were no significant difference (p 〉 0. 05). The between of the confidence interval of values of bilateral 95% of ADC of HCC focuses & that of PVTT were 1.062 - 1.206 × 10-3mm2/s & 0.909 - 1.123 × 10-3mm2/s respectively. The values of ADC of the two kind of lesions showed no significant difference (p 〉 0.05). Conclusion It was similar to the findings ofADC& DWI between PVTT & that of HCC focuses & the findings of MRI can be for the MRI diagnosis of HCC & PVTT provided supplementary.
