痰热清注射液联合阿奇霉素治疗小儿支原体肺炎疗效分析

The Effect of Tanreqing Injection and Azithromycin in the Treatment of Children with Mycoplasma Pneumonia Pneumoniae

目的:探讨痰热清注射液联合阿奇霉素治疗小儿支原体肺炎的临床疗效。方法:将我院2007-2009年收治的120例支原体肺炎患儿按随机数表法分为观察组和对照组各60例,均给予止咳化痰、退热等常规综合治疗。在此基础上对照组给予阿奇霉素序贯治疗:阿奇霉素10 mg/(kg·d)静脉滴注,1次/天,连用3～5 d,病情好转后改口服,用3 d停4 d,疗程10～14 d;观察组在对照组治疗基础上,给予痰热清注射液0.3～0.5 mL/(kg·d),1次/天,每日最大剂量20 mL,静脉滴注治疗7 d。治疗期间停用其他抗菌药物。结果:(1)治疗后观察组血清IL-6、TNF-α、CRP下降水平均较对照组更明显(P<0.01);(2)观察组和对照组发热、咳嗽、肺部啰音、X线胸片阴影消失时间分别为(5.6±2.3)d vs(7.4±3.8)d、(7.1±3.5)d vs(9.3±4.1)d、(7.9±3.9)d vs(10.5±4.7)d、(12.3±4.6)d vs(15.1±5.4)d,组间比较差异均有统计学意义(P<0.05);(3)观察组和对照组总有效率分别为98.3%vs 86.7%(P<0.05),不良反应发生率分别为15.0%、18.3%(P>0.05)。结论:痰热清注射液联合阿奇霉素治疗小儿支原体肺炎较单用阿奇霉素疗效更显著,且不会增加不良反应。

Objective： To investigate the clinical curative effect of azithromycin combined with Tanreqing injection in treating children with mycoplasma pneumoniae pneumonia （MPP）. Methods： One hundred and twenty cases of children with MPP were randomly divided into an experimental group （60 cases ） and a control group （60 cases ） in our hospital from 2007 to 2009, all of then were given comprehensive therapy such as relieving cough, sputum and fever. The control group was treated with azithromycin 10 rag/（ kg · d） , once a day, lasts 3 N 5 days; the experimental group was treated with azithromycin combined with Tanreqing injection 0.3 - 0.5 mL/（ kg · d） , qd. Results： The time to defervesce, disappearance time of cough and pulmonary tales, and the time to clear lung fields on chest X-ray report in the experimental group and the control group were （ 5.6 ± 2.3 ） days vs （ 7.4 ± 3. 8 ） days, （7.1±3.5） daysvs （9.3±4.1） days, （7.9±3.9） daysvs （10.5±4.7） days, and （12.3±4.6） daysvs （15.1±5.4） days. The differences were statistically significant （P〈0.05）. The total effective rate of the experimental group （98.3%） was significantly higher than that of the control group （86.7%） ; there were significant differences between the two groups （P〈0.05）. Conclusions： The effect of azithromycin combined with Tanreqing injection in treating children with mycoplasma pneumoniae pneumonia is better than that of only azithromycin. It is worth promoting.
