儿童原发性血小板减少性紫癜中药治疗的用药规律研究

Research on Drug Law for Chinese Medicine Treatment Treating Children Idiopathic Throbocytopenic Purpura

目的:研究中药复方治疗儿童原发性血小板减少性紫癜(ITP)的用药规律。方法:检索CNKI建立数据库,应用统计学方法分析高频方药及其性味归经,探索儿童ITP中药处方规律和特点。结果:所有处方共应用中药153味,748频次;高频药物以补虚类、清热凉血类为主;性味多为苦寒药,其次为甘寒和甘平药物;归经多为肝经、心经,其次为脾经和肺经;功效以清热凉血为主,其次为健脾益气生血。总体疗效比较,中药治疗组明显高于对照组(92.03%:72.24%,P<0.05)。结论:儿童ITP中药治疗以苦寒类药最为常用,以清热凉血为主要治则,兼用味甘平的补虚类药物,以健脾益气生血为治则,这与其发病机制密切相关;深入研究处方规律,根据发病规律正确辨证论治,对于提高临床疗效具有很好的指导意义。

Objective ：To study the drug law for Chinese medicine treatment of children with idiopathic thrombocytopenic pur- pura （ITP）. Methods：We searched CNKI database,constructed the Chinese medicine treatment database for children with ITP, analyzed high frequency prescriptions as well as high - frequency drug of tastes, meridians, and explored the drug law and charac- teristics. Results：In all prescriptions, there were 153 kinds Chinese herbs ,748 frequencies. The high- frequency drugs were tonic class and heat - clearing and blood - cooling class. Taste was respectively bitter cold, sweet clod and sweet light and meridians re- spectively the liver, heart, spleen and lung. The most efficacy was removing pathogenic heat from blood, followed strengthening spleen and supporting Qi, enriching blood. The overall efficacy of the treatment group was significantly higher （92.03% ：72. 24% ） than that of the control group. Conclusion：Chinese medicine treatment for children with ITP most commonly used the bitter cold class, the main therapeutically principle was removing pathogenic heat from blood, followed the sweet light class, the main therapeutically principle was strengthening spleen and supporting Qi, enriching blood. These drug laws were closely related to pathogenesis. Making the drug law research according the pathogenesis and then correctly determining treatment based in patho- zenesis obtained throuzh differentiation of symptoms and signs was very meaningful and could improve the therapeutic effect.
