大承气汤治疗重症急性胰腺炎的疗效及对患者血清TNF-α、IL-6、IL-8的影响研究

Curative Effect of Large Chengqi Decoction in Treatment of Severe Acute Pancreatitis and Study on Influence of Serum TNF - α,IL - 6 and IL - 8

目的:观察大承气汤治疗重症急性胰腺炎(SAP)的临床疗效,及其对患者TNF-α、IL-6、IL-8的影响。方法:将87例患者随机分为观察组(45例)和对照组(42例)。两组均予常规西医对症支持治疗,观察组同时加用中药大承气汤治疗。两组疗程均为7d,观察临床疗效及TNF-α、IL-6、IL-8变化情况。结果:观察组、对照组总有效率分别为93.33%、73.81%;组间临床疗效比较,差异有统计学意义(P<0.05)。观察组WBC和AMS恢复正常时间、腹痛缓解时间及住院天数均短于对照组,差异均有统计学意义(P<0.05)。两组治疗前后组内比较,TNF-α、IL-6、IL-8差异均有统计学意义(P<0.05);组间治疗后比较,TNF-α、IL-6、IL-8差异有统计学意义(P<0.05)。结论:中药大承气汤联合西药用于治疗SAP,临床疗效良好,在改善临床症状、降低AMS、TNF-α、IL-6和IL-8、缩短病程等方面优于单纯西药。

Objective：To observe the clinical curative effect of large Chengqi Decoction in the treatment of severe acute pancre- atitis （SAP） , and to study the irffluenee of serum TNF -α, IL - 6 and IL - 8. Methods ：87 patients were randomly divided into observation group （45 cases ） and control group （42 cases）. Both groups were symptomatically supported by conventional Western medicine treatment. Observation group at the same time added Large Chengqi Decoction treatment. Both a course of two groups was 7 days. The clinical curative effect and the changes in TNF -α, IL - 6 and IL - 8 were observed. Results：The total effective rates of observation group and control group were 93.33% and 73.81%, respectively. Clinical curative effect comparison between groups had statistically significant difference （ P 〈 0. 05 ）. Afler treatment, the TNF -α, IL - 6 and IL - 8 comparisons between groups were statistically significant （P 〈 O. 05 ）. In observation group, the WBC and AMS returned to normal, abdominal pain re- lief time and hospitalization days were shorter than those of the control group, and differences were statistically significant （ P 〈 0. 05 ）. Within the two groups before and ＇after treatment group comparison, TNF -α, IL - 6 and IL - 8 had statistical significance （ P 〈 0. 05 ）. Compared between groups after treatment, the TNF -α, IL - 6 and IL - 8 had statistical significance （ P 〈 O. 05 ）. Conclusion：Chinese medicine Large Chengqi Decoction comhined with Western medicine for the treatment of SAP has good clini- cal curative effect in improving clinical symptoms,reducing the AMS,TNF -α, IL - 6 and IL - 8 and shortening the duration of symptoms,which was better than Western medicine ahme.
