经皮加压钢板与动力髋螺钉治疗转子间骨折文献的Meta分析

摘要 目的:通过比较经皮加压钢板(PCCP)及动力髋螺钉(DHS)固定的临床结果及安全性,明确经皮加压钢板治疗转子间骨折的特点。方法:系统地回顾2000年1月—2013年1月Medline、Embase、Sciencedirect、OVID、Cochrand Central database和中国生物医学文献数据库中的相关研究,比较PCCP及DHS治疗稳定转子间骨折的随机对照试验及准随机对照试验。结果:与DHS相比,PCCP可明显地缩短手术时间,减少失血及输血,并降低心血管事件的发生率。结论:PCCP具有明显地缩短手术时间、减少失血、降低心血管事件发生率等优势。 Objective To evaluate the application of the pereutanous compression plate in the treatment of femoral intertrochanteric fractures by comparing it with the dynamic hip screw using a meta-analysis method. Methods Medline, Embase, Sciencedirect, OVID, Cochrand Central database, CBM, and other relevant da ta-bases were searched from 2000 to 2013. Randomized controlled trials (RCT) and quasi-Randomized con trolled trials (qRCT) comparing the percutanous compression plate and the dynamic hip screw for the treatment of hip fractures were included. Clinical outcomes and complications were analyzed. Nine studies encompassing 914 patients were included. Results Meta-analysis showed that PCCP could significantly shorten the opera tive time, reduce blood loss and transfusion volume, diminish the incidence of cardiovascular events. However, no significant differences could be seen in length of hospitalization, rate of walking without help, early mortality and other complications. Conclusion PCCP can be an effective treatment of the hip fracture with some advan tages in compared with DHS.
