微创内固定联合骨诱导剂技术治疗股骨颈骨折的临床研究

Case-control study on minimally invasive treatment of femoral neck fracture with three double head compression screws and ACPC/BMP bone graft

摘要 目的探讨自固化磷酸钙人工骨(ACPC)复合骨形成蛋白(BMP)联合LCO.S.螺钉内固定治疗老年股骨颈骨折的疗效。方法采用3枚LCO.S.螺钉内固定(A组,48例)及3枚LCO.S.螺钉内固定结合ACPC/BMP人工骨植骨(B组,42例)治疗股骨颈骨折90例,观察两组短期疗效。结果79例获随访6个月-3.5年,平均21.6个月。A、B两组术后切口均一期愈合.无围手术期并发症发生。B组骨折愈合率明显高于A组,且骨折愈合时间较短。B组术后髋关节功能Harris评分优良率明显优于A组。且股骨头坏死发生率低。结论3枚LCO.S.螺钉内固定结合微创ACPC/BMP人工骨植入可缩短骨折愈合时间.提高骨折愈合率及髋关节功能。 Objective To evaluate the effect of three I.CO.S. compression screws and ACPC/BMP bone graft for the elderly femoral neck fracture. Methods Ninety patients with femoral neck fractures were divided into 2 groups. The patients in group A were treated with three I.CO.S. compression screws fixation, and the patients in group B were treated with three I.CO.S. compression screws fixation combined with ACPC/BMP bone graft. The patients in two groups were followed up from a short time and the short-term effects were studied. Results Seventy nine patients were followed up for 6 months to 3.5 years, with a mean of 21.6 months. All the patients in both groups got postoperative wound healing with A degree at first stage without complications. In accordance with Harris hip function score, the therapeutic effect in group B was better than that of group A. The fracture healing time in group B was shorter than that of group A. Conclusion Minimally invasive treatment with three I. CO.S. compression screws and ACPC/BMP bone graft is an ideal method for the treatment of the elderly femoral neck fracture.
