Endobutton弹性固定治疗急性下胫腓联合损伤的临床研究 被引量:7

摘要 目的总结Endobutton钢板弹性固定对急性下胫腓联合损伤的治疗效果,探讨其临床应用价值。方法回顾性分析2009-10—2012-04进行Endobutton钢板弹性固定治疗的28例急性下胫腓联合损伤的临床资料。于术后早期、3个月、6个月及末次随访时测量X线片下胫腓间隙(TBCS)、下胫腓重叠距离(TBOL)、内侧关节间隙(MW)。记录术后3、6个月及末次随访时AOFAS足踝功能评分。结果术后6个月时AOFAS评分(92.4±2.66)分较术后3个月时(85.3±3.52)分显著提高,差异有统计学意义(P<0.05),但与末次随访时(93.2±2.29)分比较差异无统计学意义(P>0.05)。术后早期、3个月、6个月及末次随访时TBCS、TBOL及MW差异均无统计学意义(P>0.05)。结论 Endobutton钢板弹性固定符合下胫腓联合的生物力学,具有功能恢复快的优点,是一种治疗急性下胫腓联合损伤的有效方法。 Objective To investigate the effect of Endobutton elastic fixation on acute distal tibiofibular syndesmosis injury and evaluate the clinical application value. Methods The clinical data of 28 patients with acute distal tibiofibular syndesmosis injury who were treated with Endobutton elastic fixation were retrospectively analyzed. The TBCS, TBOL, MW were detected by X-ray, and the AOFA ankle hind foot score at 3, 6 months after operation and the last follow-up were recorded. Results The average AOFAS score at 6 months after operation was (92.4 ±2.66)points, which was significantly higher than that of 3 months after operation's (85.3±3.52)points, there was significant difference (P 〈0.05), but had no significant difference with the last follow-up's (93.2±2.29)points (P 〉0.05). There were no significant differences in TBCS, TBOL and MW between early stage, 3 and 6 months after operation, and the last follow-up (P 〉0.05). Conclusion The Endobutton elastic fixation conforms to the biomechanics of distal tibiofibular syndesmosis, and has the advantage of quick recovery. It's an effective way for distal tibiofibular syndesmosis.
