老年退行性心脏瓣膜病83例临床分析

Clinical Analysis of 83 Patients with Senile Degenerative Cardiac Valvular Disease

目的探讨老年退行性心脏瓣膜病患者的临床特征。方法将83例老年退行性心脏瓣膜病患者根据有无二尖瓣钙化进行分组,并对两组的临床资料进行比较。结果 83例老年退行性心脏瓣膜病患者中,主动脉瓣受累62例,占74.69%;二尖瓣受累50例,占60.24%;三尖瓣受累仅2例,占2.41%;合并二尖瓣钙化组中晕厥、心肌梗死、心力衰竭、心源性猝死、窦房结病变明显高于对照组(P<0.05或P<0.01)。结论老年退行性心脏瓣膜病是随年龄的增加而增长的老年性疾病,主要累及主动脉瓣。一旦合并二尖瓣钙化,易导致心肌梗死、心力衰竭、晕厥、猝死。

Objective To study the clinical features of senile degenerative cardiac valvular disease in the elderly. Methods Eighty three patients with retrograde valvular disease were divided into two groups according to calcification of mitral valve,and that were compared with clinical data. Results Among 83 patients with retrograde valvular disease, the aortic valve were involved in 62 cases （74.69 % ）, the bicuspid valve were involved in 50 cases （60.24 %）, and the tricuspid valve were involved in 2 cases （2.41% ）. The ap opsychia, myocardial infarction, cardiac failure and cardiogenic sudden death in patients with retrograde valvular disease were signifi- cantly more than that in control group （P〈0.05 or P〈0.01） ,and the involvement of the sinuatrial node was more than that in con trol group （P〈0.01）. Conclusion The degenerative valvular disease in elderly mainly involved in the aortic valve. Calcification of mi tral valve could lead to mvocardial infarction, heart failure, syncope and sudden death.
