早产儿支气管肺发育不良的细胞治疗 被引量:4

摘要 支气管肺发育不良(bronchopulmonary dysplasia,BPD)是早产儿常见的慢性肺部疾病,发现之初主要见于胎龄较大的长期氧暴露和进行机械通气的呼吸窘迫早产儿,呈典型的早期肺膨胀异常及后期的肺囊样变、纤维化等病理过程.近30年来,随着产前激素、肺表面活性物质以及先进通气方式和温和治疗策略的应用,经典型BPD逐渐被以肺泡和微循环发育阻滞为病理特征的新型BPD替代.该新型BPD主要见于胎龄28周以下的极早早产儿,由于肺发育极不成熟,在炎性介质、通气损伤和氧自由基等的影响下,肺泡与肺循环的发育出现阻滞,逐渐呈现对氧或机械通气的依赖.随着近年来超(极)低出生体重儿存活率升高,新型BPD逐渐增多,不仅成为极不成熟早产儿的主要死因,对其远期预后亦影响巨大.BPD导致的心肺功能异常可延续至成年,甚至伴随终生,与脑性瘫痪(简称脑瘫)和神经发育延迟紧密相关.
