新疆地区门诊采血室护士心理健康状况调查及原因分析

Investigation of psychological state of nurses in outpatient blood collecting room and cause analysis in Xinjiang

目的了解新疆地区三级甲等教学医院维吾尔族(简称维族)及汉族门诊采血室护士心理健康状况,进行原因分析。方法随机抽取77名门诊采血室护士和67名健康成人进行汉密尔顿抑郁量表(HAMD)、汉密尔顿焦虑量表(HAMA)和症状自评量表(SCL90)心理测评,对门诊采血室护士的主要症状进行评分以了解其心理状态。结果采血室护士的HAMD、HAMA及SCL-90评分高于对照组,抑郁及焦虑心理发生率高于对照组(P<0.01)。结论采血室护士心理压力过大,主要原因为护士与患者及家属沟通障碍,技术、操作上不允许有失误,害怕医源性感染,患者易烦恼和激动,护士工作繁重、机械重复,无暇顾及患者的焦虑心情、社会上对护理工作的偏见、歧视、害怕护理纠纷和投诉、语言不通沟通障碍等。

Objective To assess psychological state and analyze the causes for psychological pressure of nurse working at outpatient blood collecting room in teaching-hospital of Xingjiang. Methods To assess the causes of psychological pressure, we randomly enrolled 77 nurses and 67 controls to perform psychological assessment by Hamilton Rating Scale for Depression （HAMI）） Hamilton Anxiety Scale （HAMA） and the self-report symptom inventory, Symptom checklist, （SCL-90）. Results The scores of HAMD, HAMA and SCL-90 were significantly increased in the group of nurse （P 〈0. 01 ）. And the factorial scores of HAMI）, HAMA and SCL-90 were also significantly increased in the group of nurse （ P 〈 0. 01 ）. Psychological pressure resulting from language disorder or lack of communication was the higher occurrence rate in Han Chinese nurse. Conclu- sion The present study suggested that nurses working at blood collecting room of outpatient with more psycho- logical pressure than healthy people. Language disorder or lack of communication can be considered as the major causes of psychological pressure in Han nationality nurses compared with Uygur nurses.
