3种肿瘤标志物联合检测在肺癌临床诊断中的价值 被引量:3

摘要 目的探讨血清人付睾上皮分泌蛋白4(HE4)、神经元特异性烯醇化酶(NSE)及细胞角蛋白片段19(CYFRA21-1)在肺癌诊断中的应用价值。方法选择手术治疗的不同组织学类型的肺癌患者50例作为肺癌组,肺良性疾病组42例作为疾病对照组,健康体检者40例作为健康对照组,采用酶联免疫吸附试验检测HE4,同时用化学发光免疫法检测NSE、CYFRA21-1。结果肺癌组HE4、NSE、CYFRA21-1检测水平均明显高于疾病对照组和健康对照组(P<0.01)。HE4检测水平在肺腺癌组呈现高表达,明显高于鳞癌和小细胞肺癌组(P<0.01)。NSE检测在小细胞肺癌组呈现高表达,明显高于鳞癌和腺癌(P<0.01)。CYFRA21-1检测在鳞癌组呈现高表达,明显高于腺癌和小细胞肺癌(P<0.01)。肺癌患者组中的HE4和NSE、CYFRA21-1的敏感性分别为44.0%和38.0%和62.0%,特异性分别为90.0%和87.5%和92.5%,HE4、NSE、CYFRA21-1联合检测的敏感性为78.0%,高于单项检测的敏感度。结论血清HE4、NSE、CYFRA21-1联合检测有助于不同组织类型肺癌的分析,而且可以明显提高肺癌临床诊断的敏感性。
