摘要
目的:探讨磷酸肌酸联合参麦治疗新生儿窒息后心肌损害的临床疗效,以及对心功能的影响。方法:收集80例新生儿窒息后心肌损害患儿,采用随机数字的方法分为观察组和对照组各40例,均给予综合治疗,在此基础上对照组加用果糖二磷酸钠和维生素C,观察组加用磷酸肌酸钠和参麦注射液,疗程均为7d。观察两组患儿临床症状及体征、心电图、心肌酶谱、肌钙蛋白及左心功能等变化。结果:观察组患儿临床总有效率为95.00%,明显高于对照72.50%,差异存在统计学意义(P<0.05)。两组患儿治疗前血清CK-MB和cTnI水平,以及LVEF和SF比较,差异无统计学意义(P>0.05);疗程结束后,两组患而血清CK-MB和cTnI水平,以及LVEF和SF均得到改善P<0.05),但是观察组变化幅度更大P<0.05)。结论:磷酸肌酸联合参麦治疗新生儿窒息后心肌损害临床疗效确切、促进心肌恢复、保护心脏功能、改善心肌能量代谢,值得临床推广应用。
Objective: To investigate the clinical therapeutic effects of phosphocreatine combined with Shenmai on cardiac damage secondary to neonatal asphyxia and the influence on heart function. Method: 80 sick infants with cardiac damage sec- ondary to neonatal asphyxia were collected and randorrdy divided into observation group and control group, 40 infants in each group; all the infants were given with combined modality therapy, additionally, the control group was supplemented with fruc- tose diphosphate sodium and vitamin C, while the observation group was supplemented with phosphocreatine and Sbenmai injec- tion, and the course of treatments of both groups were 7 days. The changes in the clinical symptom, physical sign, ECG, myo- cardial zymogram, treponin, and left heart function and so on of the infants of the two groups were observed. Resttlt: The clini- cal total effective rate of the infants of the observation group was 95.00%, significantly higher than that of the control group ( 72.50% ), and the difference was statistically significant ( P〈O. 05 ). The differences in pre-therapy serum CK-MB and cTnI levels, LVEF, and SF between the two groups were statistically insignificant (P〉0.05) ; After the treatments, the serum CK- MB and cTnI levels, LVEF, and SF of both groups were improved (P〈O. O5), but those of the observation group were im- proved more significantly ( P〈O. 05 ). Conclusion: Phosphocreatine combined with Sbenmai in treatment of cardiac damage secondary to neonatal asphyxia is effective, can promote myocardial recovery, protect heart function and improve myocardial en- ergy metabolism, and consequently is worthy of clinical application.
