活血化瘀法对结核性胸膜炎患者中医证候疗效及肺功能的影响

Effect of Huoxuehuayu Method on TCM Syndrome and Pulmonary Function in Patients with Tuberculous Pleurisy

目的 观察活血化瘀法治疗对结核性胸膜炎患者的中医证候疗效,以及对后期肺功能的影响.方法 将患者简单随机分为两组,均给予常规治疗（抗结核药物＋胸腔穿刺抽液术）,治疗组加用活血化瘀的中药,酌情治疗1～3周.之后治疗组继续口服抗结核药物＋活血化瘀中成药,对照组只给予口服抗结核药物,酌情治疗7～9周.观察两组治疗前后中医证候以及肺功能相关指标的变化情况.结果 在中医证候方面,治疗组治疗前后症状比较差异有统计学意义（P＜0.05）,对照组仅在发热,咳嗽方面有明显改善（P＜0.05）,组间比较治疗组胸胁痛、气促较对照组明显改善（P＜0.05）;治疗组总有效率明显高于对照组（P＜0.05）.两组治疗后FVC、VC均显著提高（P＜ 0.05）,FEV1/FVC％下降（P＜0.05）;治疗组VC、FEV1/FVC％改善优于对照组（P＜0.05）.结论 活血化瘀的中药能显著改善结核性胸膜炎患者中医证候,同时能够改善患者肺功能,进而改善疾病的预后.

Objective：To explore the tuberculous pleuritis patients,who using Chinese patent drug with the efficiency of Huoxuehuayu method,by the targets of the curative effect about the field of traditional Chinese medicine and the influence of pulmonary function in the patients.Methods：60 eligible patients were divided randomly into 2 groups with 30 cases in each group.While in the hospital,the treatment group was used the conventional treatment of western medicine （medicine for tuberculosis and the operation of taking out hydrothorax from the thoracic cavity）,combining with Chinese patent drug,which had the efficiency of Huoxuehuayu method.Meanwhile,the control group was used just the conventional treatment of western medicine.The course of treatment was 2 weeks more or less 7 days.After leaving hospital,the treatment group was used the medicine for curing tuberculosis and Chinese patent drug,which had the Huoxuehuayu method.The control group was just used the western medicine for curing tuberculosis.The course of treatment was 8 weeks more or less 7 days.The observing targets including therapeutic effect of traditional Chinese medicine and pulmonary function were observed and compared between 2 groups before and after the therapy.Results：In the TCM syndrome,there was an obvious difference before and after treatment in the treatment group （P＜ 0.05）.Meanwhile,the fever and the cough were improved remarkably after treatment in the control group（P＜0.05）.Some indicators of the treatment group were superior to those of the control group such as the total effective rate and the improvements including the short breath and the chest and flank pain.In the pulmonary function indicators,the FVC and the VC were improved obviously in 2 groups.However,the FEV1/FVC in 2 groups was reduced significantly （P＜ 0.05）.Also,improvements such as the VC and the FEV1/FVC were more obviously in the treatment group than those in the control group （P ＜ 0.05）.Conclusion：Chinese medicine with Huoxuehuayu method can not only effectively improve the clinical symptom,but also increase the pulmonary function.It is further to improve the survival of the patients.
