血液透析老年患者心理状况与自我管理相关性研究

Relationship between psychology and self-management among elderly patients treated with hemodialysis

目的了解维持性血液透析老年患者心理健康状况,探讨其与自我管理行为的相关关系。方法采用老年人心理状态量表和自我管理量表,对西安市4所三级甲等医院血液净化中心的164例维持性血液透析的老年患者进行调查。结果患者心理状况总得分在43-138之间,总均分为(101.7±21.0)分;经常看新闻及经常与好友联系的患者心理状况得分较高,差异有统计学意义(P=0.003);患者心理状况与自我管理得分呈正相关(r=0.486,P<0.01);患者心理状况的影响因素为运动情况、与医生沟通(P<0.05)。结论医护人员应给予透析的老年患者心理支持,包括运动指导和透析知识指导,提高患者自我管理技能,以增强患者心理健康状况。

Objective To understand the psychological status among elderly patients treated with maintenance hemodialysis,and to explore its relationship with the self-management.Methods A total of 164 elderly patients treated with maintenance hemodialysis were recruited from dialysis centre of 4 top-notch hospitals in Xi＇an city,and all were investigated by psychological health questionnaire for aging and the self-management scale.Results The total score of psychology in elderly patients treated with hemodialysis was 43-138,and averaged 101.7 ± 21.0.Patients who often watched the news and contacted with friends got higher scores in the mental status questionnaire （P =0.003）.The psychology scores were positively correlated to self-management （r =0.486,P ＜ 0.01）.Sport management and talking with healthcare providers influenced the psychological status （P ＜ 0.05）.Conclusion In order to enhance their self-management skills and psychological status,it is advisable for medical staffs to provide psychological support for elderly patients,including movement skill guide and hemodialysis knowledge.
