超声骨刀联合高速涡轮钻拔除复杂下颌阻生智齿的临床护理

Nursing of the complicated cases of mandibular impacted wisdom tooth extraction by combined application of piezosurgery unite and high speed turbine handpiece

目的探讨超声骨刀拔除复杂下颌阻生智齿的效果及护理经验。方法选择90例下颌阻生智齿患者,随机平均分为三组:(1)A组,采用涡轮钻法拔除;(2)B组,采用超声骨刀法拔除;(3)C组,采用超声骨刀结合涡轮钻法除。观察三组患者手术时间、术后疼痛、肿胀程度等情况。采用SPSS 13.0软件包进行统计学分析。结果 C组拔牙时间低于A、B组,差异有统计学意义(t A=25.49,tB=9.44,P均<0.05),且并发症较低(3%)。结论超声骨刀结合涡轮钻法的应用,可明显提高工作效率,减少术后并发症,降低手术风险,优质的护理配合及熟练的"六手操作"是超声骨刀结合涡轮钻法微创、高效拔除复杂下颌阻生智齿的重要保障。

Objective To evaluate the effcect of the application of Piezosurgery osteotomy in extraction of impacted mandibular third molar and summarize the nursing experiences. Methods Ninety patients with impacted wisdom teeth were randomly and evenly divided into three groups and the teeth were extracted by 3 different methods： Group A by high speed turbine handpiece, Group B by piezosurgery device, and Group C by piezosurgery device combined with high speed turbine handpiece. The surgery time, degree of pain and facial swelling after surgery were compared. All statistical analysis was performed using SPSS 13.0 software package. Results The surgery time of group C were significantly shorter than that of group A and group B （tA=25.49, ts=9.44, P 〈 0.05）, and the complication rate was also lower （3%）. Conclusions High quality nursing care and skilled six-handed technique are important guarantee for the efficient and minimally-invasive extraction of complicated impacted mandibular wisdom teeth.
