摘要
目的研究甘草酸二铵与黄芪注射液对急性肺损伤患者的临床治疗效果。方法采用前瞻性随机对照研究方法,将60例成人急性肺损伤患者分为治疗组和对照组,每组30例。对照组按照最新指南要求进行综合治疗,治疗组在综合治疗基础上应用甘草酸二铵与黄芪注射液(甘草酸二铵150 mg+黄芪20 mL注射液,每日一次),分别在治疗前、治疗后3 d和7 d测定两组患者氧合指数(PaO2/FiO2)、肺泡动脉氧分压差(A-aDO2)、乳酸(Lac)、C-反应蛋白(CRP)。结果治疗3 d和治疗7 d后治疗组比对照组PaO2/FiO2明显提高,Lac、CRP明显下降(P<0.05),治疗7 d后A-aDO2也明显下降(P<0.05)。结论甘草酸二铵与黄芪注射液对急性肺损伤患者有改善肺泡气体交换、氧合指数、组织代谢和减轻患者炎症反应的作用。
Objective To study the clinical effect on acute lung injury treated with DiammoniumG- lycyrrhizinate and AstragalusMembranaceus Injection. Methods The prospective randomized controlled study was adopted. Sixty adult patients of acute lung injury were divided into a treatment group and a control group, 30 cases in each one. In the control group, the comprehensive therapy was applied in light of the latest guideline requirements. In the treatment group, on the basis of the comprehensive therapy, DiammoniumGly- cyrrhizinate and AstragalusMembranaceus Injection were used(DiammoniumGlycyrrhizinate 150 mg + Astra- galusMembranaceus Injection 20 mL, once every day). The oxygenation index ( PaO2/FiOz ), alveolo - arterial oxygen partial pressure difference (A -aDO2 ) , lactic acid (Lac)and C -reactive protein (CRP)were deter- mined before treatment,and in 3 and 7 days after treatment in the two groups separately. Results PaO2/ TiO2 was improved apparently in 3 and 7 days after treatment in the treatment group, andthe levels of Lac and CRP were decreased apparently as compared with those in the control group ( P 〈 0.05 ). The level of A - aDO2 in 7 days of treatment was decreased apparently( P 〈 0.05 ). Conclusion DiammoniumGlycyrrhizinate and AstragalusMembranaceus Injection improve alveolar gas exchange, oxygenation index and tissue metabo- lism;and alleviate inflammatory reaction in the patients of acute lung iniury.
