十二指肠残端漏5例临床分析

Clinical analysis of duodenal stump leakage(Report of 5 cases)

目的:探讨胃癌术后出现十二指肠残端漏的原因、临床表现及预防、治疗方法。方法:对5例十二指肠残端漏的发生、表现和治疗进行分析,提高对胃癌术后十二指肠残端漏认识。结果:十二指肠残端漏的发生原因和残端病变情况、病人的一般状况及处理方法不当有关;临床表现与十二指肠残端漏发生的时间、漏口的大小有关,根据临床表现选择治疗方法。本组5例发生于术后5～9d,以局限性腹膜炎为主要表现,均经腹腔引流为主的非手术治疗治愈。结论:正确处理十二指肠残端可减少残端漏的发生;残端漏发生后,可根据具体的情况采取不同的治疗方法。

Objective：To explore clinical manifestations and prevention, treatment method for gastric cancer after duodenal stump leakage. Methods ： I Section 5 cases of duodenal stump leakage occurred, performance and treatment analysis, literature review, improve postoperative gastric cancer duodenal stump leakage understanding. Results ：Duo- denal stump leakage causes and stump lesions, the patient＇s general condition and treatment related to inappropriate; the time the clinical manifestations leakage occurred, the size of the drain port, optional treatment based on clinical manifestations. 5 cases occurred after 5~9 days, mainly localized peritonitis, abdominal drainage were confirmed by the main non-surgical treatment to cure. Conclusion：Correctly handling of the duodenal stump can reduce the inci- dence of stump leakage; different treatment can be given to the stump fistula according to specific circumstances.
