摘要
目的:探讨鼻塞持续气道正压通气(NCPAP)联合肺表面活性物质治疗新生儿呼吸窘迫综合征(NRDS)的临床疗效及安全性。方法:回顾分析NCPAP联合肺表面活性物质治疗的24例患儿及NCPAP联合盐酸氨溴索治疗的20例患儿临床资料,并比较二者的临床疗效。结果:观察组患儿总有效率为95.83%,明显高于对照组的75.00%,二者差异存在显著性(P<0.05)。治疗后2h两组患儿血气分析指标较治疗前均明显好转(P<0.05),但是观察组患儿改善更明显(P<0.01)。观察组患儿症状缓解时间、机械通气时间和住院时间均明显短于对照组,差异有统计学意义(P<0.05)。观察组并发症发生率为20.83%,明显低于对照组的50.00%(P<0.05)。结论:NCPAP联合肺表面活性物质治疗NRDS可以显著提高临床治疗效果,改善血气指标,缩短住院时间,降低并发症发生率,值得临床推广应用。
Objective: To investigate the clinical effect and safety of nasal continuous positive airway pressure(NCPAP) combined with pulmonary surfactant in treatment of neonatal respiratory distress syndrome(NRDS).Method: The clinical data about 24 infants treated with NCPAP combined with pulmonary surfactant and 20 infants treated with NCPAP combined with ambroxol hydrochloride was retrospectively analyzed,and the clinical curative effects of the two regimens were compared.Result: The total effective rate of the observation group was 95.83%,which was significantly higher than that of control group(75.00%),and the difference was significant(P〈0.05).2 hours after the therapies,the blood gas analysis indexes of both groups were significantly improved when compared with those before the treatments(P〈0.05),but those of the observation group were improved more significantly(P 0.01).The symptom relief time,duration of mechanical ventilation and length of hospital stay of the observation group were significantly shorter than those of the control group,and the differences were statistically significant(P〈0.05).The complication incidence of the observation group was 20.83%,which was significantly(P〈0.05) lower than that of the control group(50%).Conclusion: NCPAP combined with pulmonary surfactant in treatment of NRDS can significantly improve the clinical therapeutic effects,improve blood gas indexes,shorten length of hospital stay,reduce the incidence of complications,and is worthy of clinical application.
