摘要
探讨急性脑梗死血清及脑脊液 (C .S .F)血管内皮细胞生长因子 (VEGF)的动态变化及临床意义。方法 :采用ELISA双抗夹心法 ,测定 32例急性脑梗死患者 48h内、7d、14d血清VEGF及 48h内、7dC .S .FVEGF的含量。结果 :急性脑梗死后各个时期 ,血清VEGF均高于健康对照组 (P <0 .0 1) ,且以 7d时更明显 ,与 48h比较P <0 .0 5 ,与 14d比较P <0 .0 1;48h内和 7d两组间C .S .FVEGF无明显变化 (P >0 .0 5 )。脑梗死面积 >2 0ml组在各个时期的血清VEGF水平均高于 10~ 2 0ml组和 <10ml组 (P <0 .0 5 ) ,而 10~ 2 0ml组与 <10ml组间血清VEGF无差异 (P >0 .0 5 )。结论 :VEGF参与了急性脑梗死的血管重建 ,有利于脑梗死缺血半暗区血供的早期恢复 ,VEGF的升高与梗死面积的大小呈正相关。 <2 0ml的脑梗死患者VEGF升高愈明显预后愈好。为今后VEGF的基因治疗提供了理论依据。
Objective: To study the motional changes between serum and C.S.F vascular endothelial growth factor (VEGF) in acute cerebral infarction (ACI), and their clinical significances. Methods: By Sandwich Enzyme Linked Immunosorbert Assay, measure levels of serum VEGF in 48h, the 7th day and the 14th day of 32 patients with ACI, and levels of C.S.F VEGF in 48h, the 7th day of 16 patients, and levels of serum VEGF of 21 control group. Results: During the different periods after ACI, the levels of serum VEGF on the 7th day are the highest of all. Compared with that of 48h ( P <0.05), and the 14th day ( P <0.01). There are no obvious changes in C.S.F VEGF between 48h and the 7th day ( P >0.05). During the different periods, the levels of Serum VEGF in area>20ml of cerebral infarction are all higher than those of group 10-20ml and group<10ml ( P <0.05), no difference between group 10~20ml and group<10ml ( P >0.05). Conclusions: VEGF involved in the angiogenesis of ACI, it benefited the early recovery of blood supply in cerebral infarction and ischemic penumbra. The increase in the area<20ml of cerebral infarction is very obvious, prognosis is excellent. It provided theory basis for gene treatment of VEGF in future
