Practice and Experience of Clinical Pharmacists Providing Medication Instructions to Patients in Cardiovascular Department

目的探讨临床药师为心内科患者提供用药指导的模式及重点。方法阐述临床药师提供心内科患者用药指导的工作模式,并对部分典型病例进行分析。结果临床药师积极为患者提供用药指导,可提高患者用药的依从性,促进药物治疗的安全、有效、合理。结论临床药师可着眼于药品不良反应、择时给药、药物相互作用宣教及重点患者随访等方面,积极开展患者用药指导。

Objective To explore the working mode and focus of patients＂ medication instructions provided by clinical pharmacists in cardiovascular department. Methods The working mode of clinical pharmacists providing medication instructions to cardiovascular patients was described, with some typical cases analyzed. Results Clinical pharmacists providing medication instructions not only improve patient compliance, but also promote the medication more safe, effective and reasonable. Conclusion To provide effective patients＂ medication instructions, clinical pharmacists can focus on disseminating adverse drug reaction, timing of administration and drug interactions, as well as following up of key patients.
