复方斑蝥胶囊联合化疗治疗老年晚期胃癌的疗效观察

Efficacy of Fufangbanmao capsules combined with chemotherapy in the treatment of elderly patients with advanced gastric cancer

目的评价复方斑蝥胶囊辅助紫杉醇(PTX)+5-氟尿嘧啶(5-FU)+亚叶酸钙(LV)化疗方案治疗老年晚期胃癌的临床疗效及不良反应。方法将86例老年晚期胃癌患者随机分为2组,联合组40例采用复方斑蝥胶囊辅助PTX+5-FU+LV方案化疗;对照组46例单用PTX+5-FU+LV方案化疗。分别于治疗前后对患者外周血白细胞计数、肝肾功能、食欲、消化道反应、体质量、Karnofsky评分、疗效进行评价。结果联合组总有效率47.5%,对照组总有效率26.1%,2组比较差异有统计学意义(P<0.05),治疗组疗效优于对照组。联合组中位生存时间11.7个月,对照组8.1个月,2组比较差异有统计学意义(P<0.05)。联合组1年生存率55.0%,对照组30.4%,2组比较差异有统计学意义(P<0.05)。联合组生活质量显著改善,与对照组2组比较差异有统计学意义(P<0.05)。复方斑蝥胶囊组白细胞减少程度均好于对照组(P<0.05)。结论复方斑蝥胶囊联合PTX+5-FU+LV方案化疗可以增加疗效,减轻骨髓毒性,提高患者生存质量和化疗耐受性,值得临床推广。

Objective To evaluate the efficacy and side-effects in Elderly patients with advanced gastric cancer treated by paclitaxel （ PTX）, 5 - ffuorouracil （ 5 - FU ） and Leucovorin （ LV ） with Fufangbanmao capsules or without Fufangbanmao capsules. Methods 86 elderly patients with advanced gastric cancer were divided into two groups with a randomized,parallel control approach. In the treatment group,40 patients were treated with PTX ＋ 5 - FU ＋ LV ＋ Fufangbanmao capsules. In the control group 46 patients were treated by treatment group＇ s therapy except Fufangbanmao capsules. White blood cell count,liver and kidney function, loss of appetite, gastrointestinal reactions, body weight, Karnofsky score and efficacy were evaluated before and after treatment. Results The total effective rate was 47.5% in the treatment group and 26.1% in the control group. The total effective rate in treatment group was superior to that in control group（ P 〈 0.05 ）. The median survival time in treatment group（ 11.7 month） was longer than that in control group（8.1 month,P 〈 0.05 ）. The one - year survival rate in treatment group （55.0%） was longer than that in control group（ 30.4%, P 〈 0.05 ）. The quality of life in treatment group were more significantly improved than those in control group（P 〈 0.05 ）. The frequency of leukocyte reduction in treatment group were better than in control group（P 〈 0.05 ）. Conclusion Fufangbanmao capsules combined with PTX ＋ 5 - FU ＋ LV regimen can improve the therapeutic efficacy, reduce the chemotherapy - related toxicities of digestive tract and bone marrow, and improve the quality of life and the tolerance of chemotherapy of elderly patients with advanced gastric cancer.
