内镜引导下口内切口单端固定治疗颧弓骨折的临床研究

CLINICAL STUDY ON ENDOSCOPE-ASSISTED SINGLE ENDED FIXED TREATMENT OF ZYGOMATIC ARCH FRACTURE USING INNER ORAL INCISION

目的研究内镜引导下口内切口治疗颧弓骨折单端固定的临床效果。方法对52例颧弓骨折患者进行手术治疗,在上颌后牙相应部位的前庭沟做小切口入路,内镜监控下确定颧弓骨折线位置后,适当调整两侧骨断端,将移位的骨折断端按解剖关系准确复位。在靠近颧骨的颧弓骨折断端行单端固定。通过手术效果、有无并发症及患者满意度调查等对此手术方法进行评价。结果 52例患者手术全部60min内结束,术后切口均无感染、血肿及气肿。口内切口瘢痕隐蔽。全部患者颧部对称,颧牙槽嵴等骨折部位无骨性台阶形成,侧压颧部有极好的稳定性。结论内镜引导下行颧弓骨折复位固定术,避免了传统方法单凭感觉复位导致断端复位不齐的弊端,并且口内切口加以单端固定,保证了复位后的稳定性及美观性,并有效减轻患者术后并发症。

Objective To study the clinical effect of endoscope-assisted single ended fixed treatment of zygomatic arch fracture using inner oral incision.Methods Fifty-two patients with fracture of zygomatic arch were as subjects who accepted surgical therapy.The point of inner oral incision was located at vestibular groove corresponding to maxillary molar.After the position of fracture line was ascertained with the assistant of endoscope,both ends of the bone fracture were adjusted,the displaced fracture ends were accurately reset.Then,single ended fixture was performed near the fracture ends of the zygomatic arch.Results The operation time was within 60 minutes in all 52 patients.Postoperative wound of all patients healed without infection,hematoma and emphysema.Intraoral incision hidden scars had no aesthetic impact.The fracture site was flat,symmetrical and stable.Conclusion The operation can increase the stable and aesthetic impact and reduce complications which avoid the disadvantages of traditional treatment.
