基层医院腹腔镜胆囊切除术并发症的预防和处理

Prevention and treatment of complications in laparoscopic cholecystectomy in primary hospital

目的探讨基层医院腹腔镜胆囊切除术(LC)并发症的预防和处理对策。方法回顾性分析该院65例行LC患者的临床资料。结果 64例(98.5%)顺利完成LC,中转开腹1例(1.5%);出现并发症3例(4.6%),其中胆管损伤1例(1.5%),术中术后出血1例(1.5%),肠道损伤1例(1.5%),经针对性处理均治愈出院。结论应从思想上加强腹腔镜手术操作的风险认识,严格选择手术病例和适应证,术中做到规范和精细操作,可最大限度地预防和减少LC术并发症的发生。

Objective To investigate the prevention and treatment of operative complication in laparoscopic eholeeystectomy（LC） in primary hospital. Methods The clinical data of 65 cases received LC were analyzed retro- spectively. Results Operation was successfully performed in 64 eases（98.5% ） and one case was converted to lapa- rotomy（1.5% ）; 3 patients suffered from complications （4. 6% ）, which were corroborated that one patient was the bile duct injury（ 1.5% ）, another was the intraoperative and postoperative hemorrhage（ 1.5% ） and the other was the intestinal tract injury （ 1.5% ）. These 3 patients all recovered after laparoscopic repair operation. Conclusion Strengthening the thought on laparoscopie operation risk awareness, strictly controlling operation indications and choo- sing cases, ensuring intraoperative standard and fine operation can inaximally rednce or even prevent the incidence of complications of LC.
