艰难梭菌感染实验室诊断的研究进展 被引量:4

摘要 艰难梭菌(C d )是革兰阳性厌氧芽孢杆菌,能引起伪膜性肠炎等感染性疾病(CDI)。近年来随着抗生素的广泛使用和NAP1/027克隆株在世界范围内传播,CDI发病率和严重程度不断增加。在加拿大、美国和英国相继导致医院内及社区流行,已被公认为发达国家重要的医院内感染病原菌之一[1]。住院患者长期使用抗生素可杀灭肠道内敏感细菌,破坏肠道微生态平衡,导致Cd过度生长并释放毒素,与CDI密切相关的毒素包括T cdA和T cdB。美国感染病协会2010年版的CDI诊断标准包括:(1)有腹泻症状,24 h内排未成形便大于或等于3次;(2)大多数患者近8周内使用过抗生素;(3)粪便中检出产毒型Cd或Cd毒素,或结肠镜检或组织病理检查发现伪膜性肠炎[2]。其中腹泻症状与抗生素使用为非特异指标;病理学检查为有创操作,不适于常规开展。因此,准确、快速的实验室检验结果对CDI诊断和治疗具有重要意义。现有的CDI检测方法种类较多,它们各自有优势和局限性,本文对CDI实验室诊断的研究进展作一综述。
