动脉瘤夹位置与动脉瘤模型的关系 被引量:2

摘要 目的通过调整动脉瘤夹位置,提高弹性酶结合动脉瘤夹法兔囊状动脉瘤模型的成功率。方法将50只雄性新西兰大白兔随机分为A、B、C组,采用弹性酶结合动脉瘤夹法建立兔囊状动脉瘤模型;A组20只采用弧形无创动脉瘤夹临时夹在右侧颈总动脉起始部及部分右侧锁骨下动脉上,B、C组各15只动脉瘤夹内侧缘分别夹在离起始段0.3 cm和1.0 cm处。造模第28天,磁共振血管成像(MRA)检查该模型动脉瘤形态,弹性纤维染色法观察其病理学变化。结果造模第28天,MRA显示A组15只可见动脉瘤影,B组仅1只见窄颈动脉瘤,C组均未见明显动脉瘤影。A组与B、C组比较,P均<0.01。A组动脉瘤壁增厚,内皮细胞消失;弹力纤维明显减少、甚至消失,外层胶原纤维致密增厚;炎性细胞减少。而B、C组为血管残端,组织学表现为内皮细胞及弹力纤维完整,少量炎性细胞浸润。结论动脉瘤夹内侧缘的位置在右侧颈总动脉起始段,能明显提高弹性酶结合动脉瘤夹法兔囊状动脉瘤模型的成功率。
