Expression and significance of RANTES and MCP-1 in renal tissue with chronic active antibody-mediated rejection

摘要 目的探讨慢性排斥反应时移植肾组织中调节激活正常T淋巴细胞表达和分泌的细胞因子(RANTES)、单核细胞趋化蛋白-1(MCP-1)和Ⅳ型胶原的表达,并分析三者与移植肾慢性排斥反应肾间质纤维化/小管萎缩(IF/TA)的相关性。方法选择40例病理诊断符合慢性排斥反应的受者,采用免疫组织化学技术和计算机真彩色图像分析系统半定量检测移植肾组织中RANTES和MCP-1的表达,并分析其表达与发生慢性排斥反应时移植肾组织IF/TA的关系。以10例正常肾组织作为对照。结果发生慢性排斥反应时,移植肾组织中RANTES、MCP-1和Ⅳ型胶原的表达均较正常肾组织明显增加(P〈0.01),并随着IF/TA病理分级的增加而呈递增的趋势,RANTES和MCP-1的表达与IF/TA病理分级呈正相关(相关系数分别为0.940和0.954,P〈0.01)。结论在发生慢性排斥反应患者的移植肾组织中,RANTES和MCP-1的高表达与移植肾纤维化和慢性移植肾功能丧失的发展密切相关。 Objective To investigate the expression of RANTES, monocyte chemoattractant protein 1 (MCP-1) and collagen IV in renal allografts with chronic active antibody-mediated rejection (ABMR), and explore its relationship with ABMR-induced interstitial fibrosis and tubular atrophy (IF/TA). Method Immunohistochemical assay and computer-assisted genuine colored image analysis system were used to detect the expression of RANTES, MCP-1 and collagen IV in renal allografts with ABMR. The relationship between expression level of MCP-1, RANTES, collagen 1V and the grade of inflammatory cell infiltration, interstitial fibrosis and tubular atrophy in renal allografts was analyzed. Ten specimens of healthy renal tissues were used as controls. Results Compared to the normal tissues, the expression levels of RANTES, MCP-1 and collagen IV were significantly increased in the renal tissues with ABMR (P^0. 001), and tended to increase along with the pathological grade of IF/ TA. The expression of RANTES and MCP-1 was positively correlated with the pathological grades of IF/TA (r = 0. 940 and 0. 954, respectively, P(0. 001 for both). Conclusion In renal allograft tissues with ABMR, the up-regulated expression levels of RANTES and MCP-I may be related to the progression of the chronic renal allograft dysfunction and allograft fibrosis.
