高危孕产妇院内感染219例分析

Investigate the clinical characteristics of 219 cases with high-risk maternal's nosocomial infection

目的探讨高危孕产妇院内感染的发生率及高危因素。方法回顾性分析广州市重症孕产妇救治中心2009年1月至2011年12月6614例高危孕产妇中219例发生院内感染的孕产妇的临床特征。结果高危孕产妇院内感染发生率(3.31%)较同期普通孕产妇院内感染发生率(1.78%)高,差异有统计学意义(P<0.01)。高危孕产妇中,3年间非本地户籍人口组的院内感染发生率(分别为4.48%、3.96%、4.79%)均高于本地户籍人口组院内感染发生率(分别为2.49%、2.16%、2.66%),差异均有统计学意义(P<0.05)。3年间手术分娩组院内感染发生率(分别为4.27%、3.72%和4.13%)均高于阴道分娩组院内感染发生率(分别为2.19%、2.02%、2.34%),差异均有统计学意义(P<0.05)。院内感染中最常见为上呼吸道感染,占30.13%;其次为生殖道感染,占26.02%。结论高危孕产妇的院内感染发生率高于普通妊娠孕产妇,且以上呼吸道感染及生殖道感染最常见,非本地户籍人口及手术分娩可能是高危孕产妇发生院内感染的高发因素。

Objective To investgate the high-risk maternal nosocomial infection incidence rate and the risk factors. Methods 219 cases of nosocomial infection in 6 614 high-risk patients from Jan 2009 to Dec 2011 were retrospetively analyzed. Results The high-risk maternal nosocomial infection incidence rate （3.31%） was higher than the normal maternal nosocomial infection incidence rate （1.78%）.There was a statistcs difference between them （P〈0.01）. From 2009 to 2011 ,every year the nosocomial infection＇s incidence rate of non-local high-risk patients （4.48% ,3.96%, 4.79%） was higher than the nosocomial infection＇s incidence rate of local high-risk patients （2.49% ,2.16% ,2.66% ）. There was a statistics difference between them （P〈0.05）. The nosocomial infection＇s incidence rate of operational delivery （4.27%,3.72%,4.13%） was also higher than the nosocomial infection＇s incidence race of vinginal delivery （2.19%, 2.02%, 2.34%）. There was a statistics difference between them（P〈0.05）. The most common nosocomial infection site of high-risk patients was the upper respiratory tract infection （50.13%）, follow it was the reproductive tract infection （26.02%）. Conclusions The high-risk maternal nosoeomial infection incidence rate was higher than that of normal pregnant women. The non-local resident and operational delivery maybe the risk factors of high-risk maternal nosocomial infection. The upper respiratory infection and reproductive tract infections were the main infection sites.
