联合入路双或三钢板固定治疗SchatzkerⅤ型及Ⅵ型胫骨平台骨折 被引量:14

摘要 目的探讨联合入路双或三钢板固定治疗SchatzkerⅤ、Ⅵ型胫骨平台骨折的方法及疗效。方法2007年1月至2010年1月共收治38例(38膝)SchatzkerⅤ、Ⅵ型胫骨平台骨折患者,男21例,女17例;年龄17~61岁,平均37.7岁。骨折按Schatzker分型:Ⅴ型28例,Ⅵ型10例。骨折按AO分型:41-c1型7例,41.c2型3例,41-c3型28例。将所有患者的膝关节cT扫描数据(.dicom格式)导人数字化骨科临床研究平台系统(SuperImage1.2),自动模拟患者骨骼情况,从而模拟术中复位、辅助制定术前手术计划。根据伤情选择一期外固定支架固定、二期内固定或直接内固定术。术后记录患者骨折愈合、膝关节活动度、美国特种外科医院膝关节评分(HSS)等。结果所有患者术后获9~36个月(平均17.7个月)随访,骨折临床愈合时间为3-9个月(平均5.6个月)。术后9个月屈膝75°~120°(平均91.3°),伸膝0~30°(平均5.1°),HSS评分为62~91分(平均80.1分)。术后所有患者双侧下肢长度相差不超过1cm,胫骨力线恢复正常。术后3例患者出现胫骨平台骨折畸形愈合,2例患者出现术后切口坏死。结论对于SchatzkerⅤ、Ⅵ型胫骨平台骨折术前应明确骨折块情况,制定良好的手术方案,突出治疗的分期性及个体化,利用联合入路获得充足视野,尽可能恢复下肢力线、长度及关节面平整,并利用双或三钢板行稳定固定,可取得满意疗效。
