使用微螺钉种植体压低上前牙改善露龈笑的效果观察

Intruding Upper Anterior Teeth Using Mini-Screw Implant to Improve Gummy Smile

目的评价应用微螺钉种植体压低上前牙以改善露龈笑的临床效果。方法选择11例露龈笑伴前牙深覆骀患者，其中男性1例，女性10例，年龄18．6～24．3岁，平均21．2岁，在上颌双侧侧切牙与尖牙间唇侧植入微螺钉种植体，配合全口固定矫治器，以种植体支抗弹性牵引的方式压低上前牙，力量约为每牙30-50g，压低时间平均约为7个月，治疗前后拍摄全口曲面断层片进行测量对比，观察前牙的位置变化和露龈笑改善的程度。结果11例患者治疗后上颌前牙平均压低3．8mm，深覆牿得到有效的纠正，露龈笑明显改善。结论利用微螺钉种植体支抗能有效地压低上前牙改善露龈笑，实现了前牙的绝对压入，避免了使用传统支抗措施压低前牙时的副作用，是一种纠正深覆牿改善微笑曲线的较好方法．值得在临床上推广。

Objective To evaluate the clinical effect of mini-screw anchorage on intruding upper anterior teeth in patients with gummy smile. Methods 11 patients with gummy smile aged 18.6 to 24.3 years old, average 21.2 years of age were in- volved in this study. The mini-screws were placed between upper lateral incisors and canines on both sides together with fixed ap-pliance to intruding upper incisors, the panoramic radiographs were taken before and after treatment to measure and compare the degree of gummy smile correction and incisor intrusion. Results Upper anterior teeth of 11 patients were intruded by 3.8 mm on average after treatment, deep overbite and gummy smile were significantly improved. Conclusion Using mini-screw anchorage to intrude the upper anterior teeth can effectively improve gummy smile, thus achieve the purpose of deep overbite correction by ab-solute intrusion, which avoid the side effects of traditional anchorage measures ,it was a better method to improve the smile curve.
