摘要
目的探讨三维时间飞跃(3D time of flight,3D—TOF)序列对血管压迫性面肌痉挛及三叉神经痛的诊断价值。方法回顾性分析45例面肌痉挛及7例三叉神经痛患者MR影像表现,并与手术结果对照。结果3D—TOF—MRA检查发现45例面肌痉挛及7例三叉神经痛患者均可找到责任血管。52例患者有症状侧均行手术治疗,其中1例三叉神经痛患者未找到责任血管,其余病例术中证实均有血管压迫或接触相应神经。经统计学分析,患者存在症状与血管是否压迫或接触神经有显著相关性(P〈0.05)。结论3D—TOF—MRA对血管压迫性面肌痉挛及三叉神经痛的诊断具有重要价值,对患者术前评估与制定手术治疗方案具有重要意义。
Objective To evaluate the value of flnee - dimensional time of flight MR angiograph(3D - TOF-MRA)in the diagnosis of hemifacial spasm and trigeminal neuralgia. Methods Total of 45 patients with hemifacial spasm and 7 patients with trigeminal neuralgia underwent 3D - TOF- MILk.The source images were analyzed and also compared with the operation re-suit. Results Results of the 3D - TOF - MRA examination revealed that 45 patients with hemifacial spasm and 7 patients with trigeminal netmalgia can find the offending vessels. All patients underwent microvacular decompression surgery, wlfich was not found in a patient with trigeminal neuralgia responsible blood vessels,the remaining cases were found flint the appropriate nerve were all compressed or contacted by surrounding vasculars. By statistical analysis, a significant colleration was found between symptoms and neurovascular compression( P 〈 0. 05). Conclusion 3D - TOF - MILA is not only valuable for the diagnosis of hemifacial spasm and trigeminal neuralgia, but also of great significance for preoperative assessment and development of surgical treamaent options.
