摘要
目的探讨丙泊酚麻醉对急性颅脑外伤手术患者血清S100B蛋白和神经元特异性烯醇化酶(NSE)和肿瘤坏死因子(TNF)-α的影响。方法选择2011年1月,2012年6月上虞市人民医院急诊行手术治疗颅脑外伤患者80例,随机分为丙泊酚组与对照组.每组各40例。丙泊酚组采用丙泊酚静脉全麻,对照组采用异氟醚吸入全麻。观察两组患者术前、术中1h、术毕时血清SIOOB蛋白、NSE和TNF-α水平的变化。结果两组患者术前血清S100B蛋白、NSE和TNF—α水平比较差异无统计学意义(P〉0.05),两组患者术中1h和术毕时血清S100B蛋白、NSE和TNF-α水平均较术前明显上升(P〈0.05或P〈0.01),且丙泊酚组上升幅度明显低于对照组(P〈0.05)。结论丙泊酚麻醉能明显降低急性颅脑手术患者S100B蛋白、NSE和TNF-α水平,降低急性颅脑损伤的程度,具有良好的脑细胞保护作用。
Objective To discuss the influence of Propofol anesthesia on serum S100B protein, neuronspecific enolase (NSE) and tumor necrosis factor-α (TNF-ct) of patients with acute craniocerebral trauma operation. Methods 80 cases of patients with acute eraniocerebral trauma emergency operation from January 2011 to June 2012 in People's Hospital of Shangyu City were selected and divided into Propofol group and control group at random with 40 cases in each group. The patients in Propofol group were given Propofol intravenous anesthesia, while the patients in control group were given Isoflurane inhalation anesthesia. The changes of serum S100B protein, NSE and TNF-α levels of patients in two groups before the operation, 1 hour after the beginning of operation, and after the operation were observed. Results The differences of preoperative serum S100 protein, NSE and TNF-α levels of patients in two groups were not statistically significant (P 〉 0.05); the S100B Protein, NSE and TNF-α levels of patients in two groups one hour during the operation and after the operation were all higher than those before operation (P 〈 0.05 or P 〈 0.01), and the rising rate in Protein group was lower than that in control group (P 〈 0.05). Conclusion Propofol anesthesia has reliable effect on reducing the SIOOB protein, NSE, TNF-α and injured levels, and brain cells protection of patients with acute craniocerebral trauma operation.
