五指山幼猪窒息心脏骤停模型复苏生存率的研究 被引量:3

摘要 目的 研究五指山幼猪窒息心脏骤停(CA)模型复苏成功率及24 h生存率,探索在复苏生存率研究中此模型的应用价值.方法 30只五指山幼猪通过在呼气末堵塞气管插管制作窒息心脏骤停模型,非干预时间8 min后行标准心肺复苏(CPR).根据自主循环恢复(ROSC)情况将30只五指山幼猪分为ROSC组与非ROSC组.计算复苏成功率,观察24h生存率状况.结果 30只五指山幼猪ROSC组12只,非ROSC组18只,复苏成功率40%.采用Kaplan-Meier乘积极限法进行生存分析,结果生存表均值(12.083±2.428)h,95% CI(7.324,16.842);生存表中位数(8.000±5.196)h,95% CI(0.000,18.184).结论 五指山幼猪窒息造成的心脏骤停模型可重复性强,成功率高,其复苏成功率与24h生存率与临床情况类似,能够满足CPR后生存率等方面的研究. Objective To research the successful ratio of resuscitation and the 24 hours survival rate of the Wuzhishan inbred mini swine model of cardiac arrest caused by asphyxia, and to explore this model application value in the survival ratio after resuscitation. Methods 30 Wuzhishan inbred mini swines underwent asphyxia by occlusion the tracheal intubation at end expiratory followed by cardiopulmonary resuscitation after 8 minutes without intervention. Then according to the difference of return of spontaneous circulation ( ROSC ) situation, the swines were divided into two groups : ROSC group and Non ROSC group. ROSC rate was calculated and 24 hours survival conditions were observed. Results Among the 30 Wuzhishan inbred mini swines, 12 were in the ROSC group and 18 were in the Non ROSC group, and the ROSC rate was 40%. Kaplan - Meier method was used to analyze the survival rate : survivorship table mean ( 12. 083 ± 2. 428 ) hours, 95 % CI (7. 324,16. 842) ; survivorship table median ( 8. 000 ± 5. 196) hours, 95 % CI (0.000,18. 184). Conclusion The Wuzhishan inbred mini swines model of cardiac arrest caused by asphyxia has good repeatability and high success rate. The successful ratio of resuscitation and the 24 hours survival rate of the swine asphyxia model are similar to those in clinic, and could satisfy the research of survival rate after cardiopulmonary resuscitation.
