小剂量盐酸右美托咪定在高血压老年全身麻醉苏醒拔管期的临床应用 被引量:5

摘要 目的观察小剂量盐酸右美托咪定在高血压老年患者全身麻醉苏醒拔管期对应激性高血压的预防作用。方法选择该院全身麻醉下实施择期手术的高血压老年患者60例,随机分为试验组(盐酸右美托咪定组)和对照组(生理盐水组)各30例。手术结束前10min分别给予微量泵输注盐酸右美托咪定0.15μg/kg(试验组)和同体积生理盐水(对照组)。观察指标:(1)分别于麻醉前(T0)、用盐酸右美托咪定或同体积生理盐水前5min(T1)、后5min(T2)、10min(T3)、拔管即刻(T4)、拔管后5min(T5)、10min(T6)、30min(T7)各时点观察血流动力学指标以及BIS值;(2)在T5、T6、T7时点进行疼痛评分;(3)拔管时间;(4)拔管期呼气末七氟烷浓度。结果 (1)在T4、T5、T6时间点,试验组MAP、HR明显低于对照组(P<0.05),(2)试验组T5、T6时点BIS评分明显低于对照组(P<0.05)。(3)2组患者疼痛评分、拔管时间以及拔管时七氟烷浓度差异无统计学意义(P>0.05)。结论在全身麻醉苏醒拔管期应用0.15ug/kg的低剂量盐酸右美托咪定在术后早期有一定镇静作用,能明显减轻高血压老年人拔管期循环波动,但不会延长患者拔管时间。
