摘要 目的:探讨肿瘤坏死因子α、脂联素对冠心病的影响及两者的相互关系。方法:采用病例对照研究将研究对象分为四组,其中病例组有3组;采用酶联免疫吸附试验(ELESA)检测血肿瘤坏死因子α、脂联素含量;并对数据进行多变量的方差分析。结果:肿瘤坏死因子α水平由高到低依次为心肌梗死合并颈/肾/脑动脉硬化组(3.34±0.92)、心肌梗死(2.72±0.86)、心绞痛组(1.63±0.54)、对照组(0.94±0.39)μg/L,其差异均具有统计学意义(P<0.05);脂联素水平由高到低依次为对照组(10.71±2.51)、心绞痛组(4.95±1.72)、心肌梗死组(3.87±1.60)、心肌梗死合并颈/肾/脑动脉硬化组(2.86±1.24)mg/L,除后两组差异无统计学意义外,其他组均差异有统计学意义;对照组、心绞痛组、心肌梗死组和心肌梗死合并颈/肾/脑动脉硬化4组间的C反应蛋白和TG,差异均有统计学意义。TNF-α和APN呈负相关(r=-0.85,P<0.001)。结论:肿瘤坏死因子α和脂联素与冠心病及其类型有关。 Objective:To analyze briefly the impact of the levels of Tumor necrosis factor-a(TNF-oL) and adiponectin(APN)on coronary heart diseases, and to explore the relationship between TNF-a and APN. Methods: Subjects were divided into four groups, three of which were cases, by case-control study. TNF-a and APN were measured by enzyme-linked immunosorbent assay (ELESA). The multivariate analysis of variance was applied by SAS version 9.2. Results: Subjects' levels of TNF-a from high to low in order were myocardial infarction combined carotid/renal/cerebral arteriosclerosis group ( MIC, 3.34 ± 0. 92), myocardial infarction ( MI, 2. 72 ± 0. 86) , angina pectoris group ( AP, 1.63 ± 0. 54) and the control group (0. 94 ± 0. 39), each of which were significantly different. The descending order of levels of APN were control group (10. 71 ±2. 51 ), AP (4. 95 ± 1.72), MI(3.87 ± 1.60), MIC (2. 86 ± 1.24), apart from the latter two groups was no statistical difference, other groups were statistically differences. The levels of C-reactive protein (CRP) and Total Glycer- in were significantly different among the four groups. TNF-c and APN was negatively correlated (r = -0. 85, P 〈0. 001 ). Conclusion: TNF-a and APN have a relationship with coronary heart disease and its relevant types.
